A New Option After Trastuzumab in HER2-Positive Gastric Cancer: A Phase III Analysis of Anbenitamab

In this article
Trastuzumab resistance in the treatment of HER2-positive gastric cancer was one of oncology's most challenging barriers. However, the KC-WISE study published in Nature Reviews is opening a new era with the "biparatopic" antibody Anbenitamab , designed to overcome this resistance. This next-generation targeted drug binds the HER2 receptor at two different sites simultaneously, pushing the boundaries of standard chemotherapy and delivering a significant improvement in survival. Offered in second-line treatment, this new option takes the vision of "personalized precision oncology" a step further in gastric cancer management.
The Clinical Need: What Should Be Done After Trastuzumab?
The standard first-line treatment for advanced HER2-positive gastric or gastroesophageal junction (G/GEJ) adenocarcinoma is trastuzumab combined with chemotherapy. However, when the disease progresses—that is, when resistance develops—the optimal second-line choice remains a matter of debate.
What Is Anbenitamab? The Secret of Biparatopic Power
Unlike conventional monoclonal antibodies, Anbenitamab has a "biparatopic" structure.
A Double-Lock Mechanism: Whereas standard antibodies bind to a single region (epitope) of the HER2 receptor, Anbenitamab binds two different epitopes on the receptor simultaneously.
The Result: This strong binding enhances the internalization and breakdown of HER2 receptors, more effectively blocking the cancer cell's growth signals.
The KC-WISE Phase III Study: Striking Data
In this randomized study of 188 patients who had received at least one line of trastuzumab-containing treatment, patients were assigned to receive Anbenitamab or placebo in addition to single-agent chemotherapy.
PFS & OS
Statistically significant improvements in progression-free survival (PFS) and overall survival (OS) outcomes.
IHC 3+ / 2+
Patients with HER2 status of IHC 3+ or IHC 2+/FISH+ were included in the study.
Safety Profile:
Anbenitamab's safety profile was found manageable when combined with chemotherapy. Treatment-related adverse effects were similar to those of other HER2-targeted agents, and no extreme increase in toxicity that would impair quality of life was observed.
Gastric cancer is a biologically heterogeneous and challenging tumor type. Anbenitamab's success has demonstrated that the "biparatopic" design can achieve superior inhibition to monoclonal antibodies. Particularly in a setting where options after trastuzumab are limited, this new option supported by phase III data renews hope for survival in patients with metastatic gastric cancer. Its future use in the first line or in combination with other immunotherapies will be an exciting area of research.
