A New Standard in Hodgkin Lymphoma Treatment: First-Line Nivolumab (Opdivo) and Chemotherapy

In this article
A historic approval and survival data that activate the immune system from the outset in the management of classical Hodgkin lymphoma.
Classical Hodgkin lymphoma (cHL) is one of the cancers with the highest cure rates, particularly in young adults. However, the long-term lung toxicity caused by standard chemotherapy regimens used for decades, particularly bleomycin-containing ABVD, was a major obstacle threatening quality of life after cure. The inclusion of nivolumab (Opdivo) in first-line treatment with a bleomycin-free regimen (AVD) increases treatment efficacy while precisely optimizing the side-effect profile. As part of our philosophy of "biological restoration," this is a triumph for the strategy of protecting the body with minimal damage while fighting cancer.
On March 20, 2026, the Food and Drug Administration (FDA), for adults and pediatric patients aged 12 years and older with previously untreated stage III or IV classical Hodgkin lymphoma (cHL), approved nivolumab (Opdivo, Bristol Myers Squibb Company) in combination with doxorubicin, vinblastine, and dacarbazine (AVD). The FDA also granted traditional approval to nivolumab for the following indications in adults with relapsed or refractory cHL:
- After autologous hematopoietic stem cell transplantation (HSCT) and brentuximab vedotin.
- After three or more lines of systemic therapy, including autologous HSCT.
Nivolumab had received accelerated approval for these indications in 2016 and 2017, respectively.
The Immunotherapy Journey in Hodgkin Lymphoma: Approval History
The evolution of nivolumab in the management of Hodgkin lymphoma is a successful scientific chronology extending from refractory cases to first-line standards:
Clinical Evidence: The SWOG S1826 Study
The efficacy of nivolumab combined with AVD was demonstrated in a randomized, open-label, multicenter study involving 994 previously untreated patients, SWOG 1826 . Patients were allocated 1:1 to nivolumab+AVD or brentuximab vedotin+AVD.
| Efficacy Parameter | Nivolumab + AVD | Comparator (BV+AVD) | Clinical Outcome |
|---|---|---|---|
| Risk of Disease Progression (HR) | 0.42 | 1.00 | A clear 58% reduction in risk (p < 0.0001) |
| Mortality Rate (Median Follow-up of 36.7 Months) | %1.8 | %3.4 | Significant survival benefit |
| Serious Adverse Events | %39 | Similar Profile | Manageable safety profile |
Administration and Dosing Protocol
The recommended nivolumab dose is 240 mg for patients weighing 40 kg or more, or 3 mg/kg for pediatric patients weighing less than 40 kg. Treatment is administered intravenously with AVD chemotherapy on days 1 and 15 of each 28-day cycle. A total of 6 cycles is intended. Primary G-CSF prophylaxis (immune support) is mandatory starting with the first cycle.
Healing Is Built Through the Right Sequence
For years, we sought a cure in Hodgkin lymphoma with "more chemotherapy." Yet the philosophy of biological restoration whispers that the true strength may lie in the body's own immune orchestra. Bringing nivolumab into the first line means tuning that orchestra correctly from the very beginning.
This approval is more than a drug approval: it is a commitment to adding long, healthy years without toxicity to our young patients' lives. Precision guidance in oncology means being able to blend the power of data with the patient's quality of life. Healing continues to rise in the compassionate hands of science.
