An Epigenetic Milestone: Revumenib (Revuforj) Receives FDA Approval for NPM1-Mutated AML

In this article
On October 24, 2025, the US Food and Drug Administration (FDA) approved the menin inhibitor revumenib (Revuforj, Syndax Pharmaceuticals) for a new indication in relapsed or refractory acute myeloid leukemia (AML) with an NPM1 mutation in adult and pediatric patients (aged ≥1 year). The approval applies to patients with no satisfactory alternative treatment.
Why Is This Approval Important?
Acute myeloid leukemia (AML) is genetically heterogeneous, with NPM1 mutation among its most common molecular alterations. Until now, however, there was no direct drug targeting the epigenetic imbalances created by this mutation.
Revumenibis the first oral treatment aiming to inhibit the menin–MLL interaction , central to DNA methylation and transcriptional control, to break the differentiation block in AML cells. Unlike conventional chemotherapy, this mechanism seeks to reprogram cellular identity —hence its description as a "second wave of epigenetic therapies."
The approval is historic not only for efficacy but also for its age range: the FDA also recorded revumenib as the first menin inhibitor usable in children aged 1 year and older .
AUGMENT-101: Revumenib Efficacy and Study Design
Revumenib efficacy was evaluated in AUGMENT-101 (SNDX-5613-0700) , an open-label, multicenter phase I/II study. It enrolled patients carrying an NPM1 mutation confirmed by next-generation sequencing or PCR , with relapsed or refractory AML in adult and pediatric populations.
- Design: Single-arm, open-label, multicenter (US- and Europe-based).
- Patient Population: NPM1 mutation-positive R/R AML, aged ≥1 year, with no alternative treatment option.
- Primary Endpoints: Complete remission (CR) and CR with partial hematologic recovery (CRh).
- Secondary Endpoints: CR+CRh duration, conversion to transfusion independence, safety profile.
📊 Clinical Results (NPM1-Mutated Population)
Revumenib produced notable remission rates in AML resistant to later-line treatments. Although obtained in a single-arm study, the combined CR+CRh rate of 23.1%is considered among the highest response levels seen in this mutation subtype.
| Parameter | Result | 95% Confidence Interval |
|---|---|---|
| CR+CRh Rate | 23.1% | 13.5 – 35.2 |
| Median CR+CRh Duration | 4.5 months | 1.2 – 8.1 |
| Conversion to Transfusion Independence | Of 46 patients, 8 (17%) | – |
Summary: Among patients treated with revumenib, approximately one-quarter achieved clinical remission , with responses lasting an average of 4.5 months . Some transfusion-dependent patients (17%) became independent with treatment. These results support the potential of epigenetic menin inhibition to produce clinically meaningful differentiation in AML.
⚠️ Safety and Warnings
- Differentiation Syndrome: One of the most serious complications; it may cause fever, edema, hypoxemia, or hypotension. It can be controlled through early diagnosis and corticosteroid initiation .
- QTc Prolongation and Torsades de Pointes: Revumenib may prolong QTc; ECG and electrolyte monitoring are recommended.
- Embryo-Fetal Toxicity: Contraindicated in pregnancy; effective contraception is mandatory for patients of reproductive potential.
The safety profile was generally manageable, with toxicities controllable through dosing-interval adjustments. Differentiation syndrome specific to menin inhibitors is also considered a biological response marker in patients responding to the drug.
Looking Ahead: A New Era of Epigenetic Therapies
Revumenib's approval marks a milestone showing that treatment in AML can target not only genetics but also the reprogramming of epigenetic memory . It heralds "epigenetic oncology" , described as cancer treatment's third major wave. Reeducating the cancer cell, rather than simply killing it, is becoming possible.
- 1️⃣ Menin inhibitors are rising: While revumenib is the first approved member of this class, other agents such as ziftomenib (KO-539) and iadademstat are progressing clinically in different AML subtypes. These drugs have potential in genetic subgroups such as MLL rearrangements (KMT2A fusions) and NPM1 mutation to be transformative .
- 2️⃣ The epigenetic-targeting paradigm: This approval demonstrates that rebalancing gene expression rather than correcting DNA mutations has entered clinical practice. Unlike conventional chemotherapy’s "hit-and-destroy" logic, it turns the "differentiate and reprogram" approach into a medical reality.
- 3️⃣ New combination possibilities: Menin inhibitors may produce deeper, more durable responses when combined with targeted agents such as venetoclax or FLT3 inhibitors . This is considered a promising direction, particularly for older patients or those ineligible for transplantation .
- 4️⃣ Potential for expansion: Revumenib is also being investigated in ongoing studies outside NPM1-mutated disease, particularly KMT2A-rearranged AML and infant leukemia . This suggests menin inhibition may extend beyond AML.
In the long term, revumenib may initiate an era of epigenetic reprogramming not only in AML but in other hematologic malignancies. This approval shows clinical research entering an era targeting cellular identity as well as mutations.
Science sometimes conceals a great revolution inside a small molecule. Revumenib may be its name— turning epigenetics into treatment and genetic information into a rewritable story , as one of the first drugs to do so.
