An Important Development from ESMO Asia 2025: For more than 20 years, oncology researchers have tried to repair damage in p53 , the gene known as the guardian of the genome. Developed by US-based PMV Pharmaceuticals, Rezatapopt (PC14586)targets the TP53 Y220C mutation and could make history in advanced solid tumors, particularly ovarian cancer, with response rates exceeding 45%.
1 Historical Context: Why the "Holy Grail"?
The p53 gene is a critical tumor suppressor that controls cell division and repairs damaged DNA, or triggers cellular suicide when repair is impossible. Scientists therefore call it the "guardian of the genome" . Yet this guardian is silenced or mutated in approximately 50% of cancers.
For years, indirect approaches such as MDM2 inhibitors were attempted without success. Directly repairing mutant p53 and restoring its function was considered almost pharmacologically impossible, or undruggable. Rezatapoptis the first-in-class molecule designed to untie this 20-year-old knot.
How Common Is the TP53 Y220C Mutation?
Statistical data on TP53 Y220C mutation frequency are as follows:
-
Frequency Across All Solid Tumors: TP53 Y220C occurs in approximately 1% of all solid tumors.
-
Share of TP53 Mutations: TP53 is the most frequently mutated gene in human cancers, with mutations in approximately 50% to 59% of solid tumors. Given its 1% prevalence across all solid tumors, Y220C accounts for the following proportion of all TP53 mutations: 1.5% to 2% in solid tumors.
Although proportionally small, its clinical impact is substantial. Y220C is a common hotspot missense mutation in p53, ranking 9th in frequency. Worldwide, this means approximately 100,000 to 125,000 new cancer cases each year involve this specific mutation.
How Does Rezatapopt Work?
The TP53 Y220C mutation creates a structural cavity in p53, destabilizing the protein and disabling its function. Rezatapopt fits into this specific cavity like molecular glue, stabilizing the protein and restoring its tumor-suppressing role. The result is cancer cell death.
Why Is It First-in-Class? A Technical Perspective
In the oncology literature, Rezatapopt is the first selective small molecule specifically targeting TP53 Y220C . Its main difference from previous p53-targeting agents (APR-246 or MDM2 inhibitors) is its mechanism as a structural corrector .
The Y220C mutation creates a small pocket or crevice on the p53 surface, causing thermal instability. Rezatapopt fits precisely into this pocket like a key. Binding stabilizes the mutant protein through refolding, allowing it to behave like wild-type p53. This degree of selectivity has not previously been seen clinically.
How Does It Differ from Other p53 Agents?
A prodrug converted to MQ inside cells, where it binds covalently to p53. It has a broad activity spectrum but does not provide Rezatapopt's specific structural filling. Its off-target effects and effects on redox metabolism remain debated.
(For example, Idasanutlin). These agents do not correct mutant p53; they aim to prevent degradation of intact, wild-type p53. They cannot reverse the loss of function caused by a specific mutation such as Y220C.
It is the clinical-stage structural corrector specific to TP53 Y220C and is the only representative of its class.
2 The PYNNACLE Phase II Study: Who Participated?
Presented at ESMO Asia 2025 in Singapore, PYNNACLE is a global basket trial.
Heavily pretreated advanced solid tumors harboring TP53 Y220C.
The largest group: Ovarian Cancer (48 patients). Also breast, colorectal and other cancers.
A convenient pill treatment taken only 3 days per week.
3 Results: Hope in Ovarian Cancer
Investigator-assessed findings are striking in a population with exhausted standard treatment options. Results in the ovarian cancer cohort are particularly notable.
Objective Response Rate (ORR) Comparison
* ctDNA Analysis: Blood tumor DNA (TP53 Y220C allele frequency) declined in every responding patient, demonstrating the drug's biological activity.

4 Safety Profile
The drug was generally well tolerated, with manageable side effects. Only 4% of patients had to discontinue because of adverse effects.
- Most Common Severe Side Effects (Grade ≥3): Elevated liver enzymes (AST/ALT 6%), anemia and thrombocytopenia (4%).
- Management Tip: Taking the drug with food substantially reduces gastrointestinal side effects such as nausea.
A New Era Begins
Rezatapopt's success represents a victory for precision medicine in oncology, beyond the success of a single drug. Patients with TP53 Y220C now have a targeted option under consideration. PMV Pharmaceuticals is preparing to test it in larger populations and combination treatments. BICR (blinded independent central review) results are also expected soon.
