A New Era in BCMA-Targeted Therapies: Belantamab Mafodotin (BLENREP) Returns with FDA Approval

On October 23, 2025, the US Food and Drug Administration (FDA) approved belantamab mafodotin-blmf in combination with bortezomib and dexamethasone for patients who had received at least two prior lines of therapy for relapsed or refractory multiple myeloma . This decision represents more than the approval of another treatment option: it also marks the end of one era and the beginning of another.

Why does it matter?

In multiple myeloma, belantamab mafodotin received FDA approval as the BCMA (B-cell maturation antigen) -targeting agent that first brought this target into clinical practice as an antibody–drug conjugate (ADC) in 2020. Shortly afterward, however, limited efficacy as monotherapy and corneal toxicity led to its withdrawal from the market. This prompted considerable debate about the future of the promising BCMA target in myeloma.

Now, compelling results from DREAMM-7 are rewriting belantamab mafodotin’s story. This study showed that, when combined with bortezomib and dexamethasone , the drug was not only better tolerated but could also triple progression-free survival . Median PFS (median progression-free survival) reaching an impressive 31.3 months established this combination as a new benchmark among BCMA-targeted therapies .

This approval is therefore viewed not only as the return of a molecule but also as a scientific milestone renewing confidence in all next-generation therapies developed along the BCMA axis . We now have a concrete example of ‘rebirth after failure’ in myeloma treatment: Belantamab mafodotin 2.0.

DREAMM-7: Belantamab Mafodotin’s Second Chance

DREAMM-7 is among the most important phase III studies designed to reassess belantamab mafodotin and demonstrate the clinical value of BCMA-targeted therapies. It enrolled adults who had received at least one prior line of therapy and had been diagnosed with relapsed or refractory multiple myeloma . FDA approval, however, was based specifically on the subgroup that had received two or more prior lines of therapy , including a proteasome inhibitor (PI) and an immunomodulatory agent (IMiD).

  • Design: Open-label, multicenter, randomized (1:1) phase III study.
  • Arms: BVd (belantamab mafodotin-blmf + bortezomib + dexamethasone) vs DVd (daratumumab + bortezomib + dexamethasone).
  • Patient Number: 217 patients (BVd n = 108; DVd n = 109).
  • Exclusion criteria: Refractoriness/intolerance to daratumumab or bortezomib, prior BCMA-targeted treatment, or significant corneal disease (only mild punctate keratopathy was permitted).
  • Primary endpoints: Time to progression (PFS) and overall survival (OS).

This study is regarded as the first large-scale evidence that BCMA ADCs can be used more effectively in synergistic combinations rather than as monotherapy . In particular, choosing the daratumumab-containing DVd arm as the control makes DREAMM-7 unique from the perspective of the ‘ADC vs antibody’ competition.

📊 Efficacy Results: DREAMM-7 Made a Real Difference

The DREAMM-7 results were striking both statistically and clinically. Patients in the belantamab combination (BVd) arm achieved approximately three times longer without progression than those in the daratumumab (DVd) arm. This difference ranks among the most marked advantages achieved to date with a BCMA-targeted agent in multiple myeloma treatment.

Endpoint BVd (Belantamab + Vd) DVd (Dara + Vd) Effect Size
Median PFS 31.3 months (95% CI: 23.5–NR) 10.4 months (95% CI: 7.0–13.4) HR 0.31 (95% CI: 0.21–0.47)
Median OS Not reached (NR) 35.7 months (95% CI: 21.1–NR) HR 0.49 (95% CI: 0.32–0.76)

In summary: Compared with the daratumumab-based regimen, the BVd combination provided a 69% reduction in progression risk and a 51% reduction in mortality risk . Median PFS exceeding 31 months stands out as the longest duration reported to date among BCMA-targeted ADCs .

⚠️ Safety Profile: Ocular (Corneal) Toxicity and the BLENREP REMS Program

Although belantamab mafodotin is an effective BCMA-targeted therapy, it also brings the most important class-specific adverse effect: corneal (eye surface) toxicity. This is associated with the drug’s carrier component, monomethyl auristatin F , a microtubule inhibitor that reaches the corneal epithelium and disrupts the cell cycle. Symptoms may include dry eyes, blurred vision, light sensitivity, and temporary reduction in vision.

  • Boxed Warning: Risk of serious ocular toxicity , including corneal epithelial changes and reduced visual acuity.
  • Incidence in DREAMM-7: Any grade: 92%, Grade 3-4 (77%) and dose modifications for ocular reasons: 83%.
  • Symptoms: Blurred vision, dry eyes, light sensitivity, and rarely temporary vision loss.
  • Pathophysiology: The microtubule inhibitor triggers apoptosis in corneal epithelial cells, disrupting the surface renewal cycle. This effect is generally reversible and improves when dosing intervals are extended.

The drug can therefore be used only with a specific monitoring protocol. BLENREP REMS (Risk Evaluation and Mitigation Strategy) requires advance certification of centers and pharmacies administering the drug. Every patient must have an eye examination before treatment and before every cycle .

  • At baseline and before every dose, visual acuity testing + corneal examination are required.
  • Artificial tears may be used routinely to prevent dry eyes.
  • If significant corneal findings develop, the dose should be delayed or reduced.
  • In addition to ocular toxicity, risks of thrombocytopenia and embryo-fetal toxicity warrant hematological monitoring and appropriate contraception.

In summary, DREAMM-7 confirmed belantamab’s strong efficacy while also showing that it ‘can be used safely with close monitoring’ . Ocular toxicity is now regarded as an aspect to manage rather than a barrier to treatment.

The belantamab mafodotin dosing regimen used in DREAMM-7 was planned in two phases to maximize efficacy while balancing toxicity . Treatment begins with a synergistic combination, followed by single-agent maintenance. This strategy aims both to reduce myeloma burden rapidly and to lower the risk of long-term adverse effects.

  • Phase 1 (Induction – First 8 cycles): Belantamab mafodotin-blmf 2.5 mg/kg is administered every 3 weeks (q3w) . During this period, bortezomib and dexamethasone are added. The aim is rapid control of tumor burden and proteasome inhibitor synergy that increases belantamab’s effectiveness.
  • Phase 2 (Maintenance – Cycle 9 onward): When the patient achieves remission or a stable response , belantamab mafodotin continues as a single agent given every 3 weeks . At this stage, bortezomib is discontinued and the risk of toxicity (particularly ocular toxicity) decreases.
  • Treatment duration: Continue until progression or unacceptable toxicity.

This two-phase approach resembles the ‘induction + maintenance’ model commonly used in myeloma. Here, however, the maintenance agent is a BCMA-targeted ADC and provides long-term target inhibition rather than persistent immunosuppression. This may create a biological bridge to future CAR-T or bispecific antibody therapies.

Detailed dosing and administration guidance for the combination’s drugs is updated on the FDA’s Drugs@FDA page.

Clinical Commentary: Who, When, and How?

Belantamab mafodotin’s reapproval establishes a new balance in multiple myeloma treatment sequencing. We now have an option involving a more accessible, intravenously administered BCMA-targeted ADC in addition to advanced technologies such as CAR-T and bispecific antibodies. DREAMM-7 shows that this agent can provide a significant advantage over conventional agents when used in the right patient and combination.

👥 Selecting Appropriate Patients

It is best suited to patients who have received at least two prior lines of therapy , including a proteasome inhibitor (PI) and an immunomodulatory agent (IMiD), who are not daratumumab-refractory and BCMA-naive , with relapsed/refractory multiple myeloma. Caution is needed in those with eye disease or advanced diabetic retinopathy.

⏱ When Should It Be Used?

The belantamab mafodotin combination is generally chosen as bridging therapy before CAR-T or for long-term control in patients not scheduled for bispecific antibodies . Earlier use is more advantageous in cases with a high tumor burden, rapid progression, or a short PFS after relapse.

🧭 How Should It Be Managed?

Treatment should be planned using an induction + maintenance approach, with regular eye examinations for ocular toxicity. If fatigue, thrombocytopenia, or blurred vision develops, the dosing interval can be extended. To assess treatment success, both M-protein and minimal residual disease (MRD) status should be evaluated.

The essence of clinical practice is to adapt treatment to the patient as much as to make the patient fit for treatment. With appropriate monitoring protocols, belantamab mafodotin is positioned as an option that is as powerful as CAR-T but more accessible . Particularly for ‘treatment-weary’ patients in later lines, it has the potential to become a new standard preserving the balance between clinical benefit and quality of life .

Looking Ahead: A New Balance in the BCMA Ecosystem

Belantamab mafodotin’s reapproval marks not only a drug’s return but also the beginning of the maturation phase of BCMA-targeted therapies . In the early 2020s, CAR-T therapies led this field; bispecific antibodies quickly followed. Both approaches, however, face challenges in manufacturing complexity, cost, and toxicity management. Antibody–drug conjugates (ADCs) such as belantamab mafodotin revive the idea of a ‘manufacturable, deliverable, and sustainable BCMA therapy’ .

  • 1️⃣ Combination Ecosystem: DREAMM-7’s success showed that ADCs can act synergistically with proteasome inhibitors and corticosteroids . Combinations with IMiDs (lenalidomide, pomalidomide) or next-generation CELMoDs are also expected to emerge.
  • 2️⃣ A New Role in Treatment Sequencing: CAR-T or bispecific antibodies are not always accessible to every patient. Belantamab is therefore being considered particularly as bridging therapy to CAR-T or as an option for ‘retargeting BCMA’ in patients who relapse after bispecific treatment. This bridging role of ADCs could transform clinical practice.
  • 3️⃣ Real-World Data and Eye Care: Beyond clinical trials, real-world data show that belantamab toxicity can be controlled through early recognition and dosing-interval management . In the coming period, optometry–oncology collaboration may become standard.
  • 4️⃣ Biological Diversity and BCMA Alternatives: As new targets beyond BCMA (such as GPRC5D and FcRH5) emerge, BCMA ADCs such as belantamab will remain a cornerstone of these multitarget strategies.

Over the long term, multiple myeloma treatment will likely evolve from ‘targeting BCMA once’ toward lifelong use of different BCMA-directed agents alternately or sequentially . In this context, belantamab mafodotin appears likely to remain the first and most accessible building block of future BCMA-based treatment architectures.

In clinical science, a molecule’s value is sometimes understood not at its first appearance but when it makes good use of its second chance . Belantamab mafodotin is writing just such a story—as a symbol of scientific persistence, the power of observation, and the courage to try again .
