A New Target in Small Cell Lung Cancer, B7-H3: Ifinatamab Deruxtecan Under FDA Priority Review
Journal of Clinical Oncology, April 2026 · Clinical Trial: NCT05280470
Small cell lung cancer (SCLC), with its aggressive course and rapidly developing resistance mechanisms after platinum-based chemotherapy, is one of oncology's most challenging areas. On April 13, 2026, the FDA, for the first B7-H3 protein-targeting ADC (antibody-drug conjugate), Ifinatamab Deruxtecan (I-DXd) , accepted the Biologics License Application (BLA) and granted “Priority Review” status. Data from the IDeate-Lung01 trial show a notable response rate of 48.2% in patients who had received multiple previous treatments.
B7-H3-Targeted Therapy: A “Smart Bomb” in SCLC Cells
B7-H3 is a protein highly overexpressed on the surface of small cell lung cancer cells and is directly associated with a poor prognosis. Ifinatamab Deruxtecan uses this protein as a “navigation system” to deliver its cytotoxic payload (DXd, a topoisomerase I inhibitor) directly into the cancer cell.
IDeate-Lung01 Trial Details
This Phase 2 trial includes patients with extensive-stage SCLC (ES-SCLC) who had received at least one line of platinum-based chemotherapy. Another notable aspect is that the 12 mg/kg dose was substantially more effective than the 8 mg/kg dose (26.1% ORR).
- 12 mg/kg Dose: 48.2% ORR and a median duration of response (DOR) of 5.3 months.
- 8 mg/kg Dose: 26.1% ORR and a median duration of response (DOR) of 7.9 months.
- Patients with Resistant Disease: Meaningful clinical activity was observed even in platinum-resistant patients who had received 2-3 previous lines of therapy.
Safety Profile: Manageable Toxicity
Findings in the safety population (n=137) at the 12 mg/kg dose resemble those of other DXd-based ADCs:
- Nausea: %48.2
- Anemia: %45.3
- Loss of Appetite: %38.7
- Neutropenia: %38.0
- Rate of Grade 3+ TEAEs (Treatment-Emergent Adverse Events): %62.0
- Serious TEAE Rate: %39.4
- Treatment Discontinuation: 10.9% (due to adverse events)
- Dose Reduction: %17.5

Unmet Need: Needs Yet to Be Met
Small cell lung cancer (SCLC) is described as a “recalcitrant” (stubborn/resistant) cancer. In patients who relapse after platinum, standard options (topotecan, lurbinectedin) generally have response rates of around 15-20%. The 48.2% response rate offered by I-DXdis virtually a quantum leap for these patients. The rapid time to response (1.4 months) may bring quick clinical relief, particularly in symptomatic patients with rapidly progressing disease.
SCLC in the ADC Era
The revolution created by HER2- and TROP2-targeted ADCs in breast cancer is now moving into SCLC with I-DXd. Widespread B7-H3 expression in SCLC could make this drug accessible to a broad population regardless of biomarker selection. The success of the 12 mg/kg dose has once again demonstrated the activity of the DXd payload in SCLC biology.
Looking Ahead
The FDA's “Priority Review” decision suggests that the drug could enter clinical use before the end of 2026. The most important question for the coming period is how much further I-DXd combined with first-line chemo-immunotherapy (PD-L1 inhibitors) will improve survival. The Phase 3 IDeate-Lung02 trial will answer these questions.
