Inluriyo (Imlunestrant) Receives FDA Approval for ESR1-Mutated Metastatic Breast Cancer

Inluriyo (Imlunestrant) Receives FDA Approval for ESR1-Mutated Metastatic Breast Cancer
In this article

The US Food and Drug Administration (FDA), on September 25, 2025 , approved Inluriyo™ (imlunestrant, Eli Lilly and Company) , a next-generation oral selective estrogen receptor degrader (SERD) agent. The drug will be used for ER-positive, HER2-negative, ESR1-mutated advanced/metastatic breast cancer in adult women and men who have received at least one prior line of endocrine therapy.

The approval is based on the phase 3 EMBER-3 study and accompanying biomarker analyses. Additionally, to detect ESR1 mutations in patients, Guardant360® CDx testi received FDA approval as a companion diagnostic . Treatment decisions will therefore be guided by ESR1 mutation positivity .

📜 The History of Oral SERDs and Imlunestrant's Place

Imlunestrant (Inluriyo)is not the first oral SERD approved by the FDA. The first approval in this class was granted in January 2023 elacestrant (Orserdu, Menarini Group) .

However, imlunestrantis the second FDA-approved oral SERD . In particular, phase 3 EMBER-3 data and approval alongside the Guardant360® CDx testi reinforce the biomarker-guided treatment paradigm.

🧬 Mechanism of Action: What Makes Imlunestrant Different?

Imlunestrant, a selective estrogen receptor degrader (SERD) , binds the estrogen receptor (ERα) with high affinity, causes a conformational change , and directs the receptor toward ubiquitin–proteasome -mediated accelerated degradation . It therefore not only weakens signaling but also reduces receptor numbers.

  • Switches off ligand-independent activity: It suppresses ER signaling driven by ESR1 mutations (e.g., Y537S, D538G) arising during aromatase inhibitor therapy, which keep the receptor continuously "on" .
  • Oral and continuous ER suppression: Daily oral use provides stable plasma levels , eliminating the fluctuations and comfort issues associated with intramuscular administration.
  • Central nervous system potential: Imlunestrant has been reported to cross the blood–brain barrier , potentially making it a rational option for patients with CNS involvement .
  • An advantage in resistance biology: ER degradation, compared with SERMs (e.g., tamoxifen), which modulate but do not eliminate the receptor, produces deeper suppression of signaling .
Class/Drug Administration Effect on ER Activity Against ESR1 Mutations CNS Penetration
SERM (Tamoxifen) Oral Modulation (antagonist/partial agonist) Limited Variable
Fulvestrant (i.m. SERD) Intramuscular (monthly) ER degradation (tissue distribution may be limited) Variable/often limited Low
Imlunestrant (oral SERD) Oral (daily) High degree of ER degradation Active in ESR1-mut+ biology Potential exists

Clinical tip: PFS benefit is most evident in the ESR1-mutated subpopulation. Therefore, ESR1 testing (liquid biopsy/NGS) becomes a critical practical step in treatment selection.

📊 EMBER-3 Study Design

EMBER-3is a phase 3, open-label, multicenter clinical study in advanced ER+/HER2− breast cancer. It enrolled 874 patients across 195 centers.

All patients had previously received an aromatase inhibitor , and some had also used a CDK4/6 inhibitor during or after that treatment. However, they were required to have received no other systemic treatment for advanced disease. This represents a population with developed resistance or disease progression .

Randomization: Patients were randomized in a 1:1:1 ratio to three arms:

  1. Imlunestrant monotherapy (400 mg orally, 1 time daily)
  2. Standard endocrine monotherapy (oral exemestane or intramuscular fulvestrant)
  3. Imlunestrant + abemaciclib combination

During randomization, patients were stratified by

  • prior CDK4/6 inhibitor use,
  • presence of visceral metastases (such as liver or lung),
  • and geographic region (North America, Europe, Asia, etc.)
to ensure balanced allocation.

Primary endpoints: Progression-free survival (PFS) —for the ESR1-mutated subgroup and the overall patient population, assessed by blinded independent central review (BICR).

Secondary endpoints: Overall survival (OS), objective response rate (ORR), duration of response (DOR), clinical benefit rate (CBR), and safety profile.

📈 EMBER-3 Findings

Among patients in the study, 38.7% had an ESR1 mutation . In this subgroup, imlunestrant provided a clear advantage over standard endocrine therapy in progression-free survival (PFS) .

Patient Group PFS (imlunestrant) PFS (control) HR (95% CI)
ESR1-mut+ (n≈256) 5.5 months 3.8 months HR=0.62 (p<0.001)
Overall population (n=874) 5.6 months 5.5 months HR=0.87 (p=0.12)
Imlunestrant + Abemaciclib 9.4 months 5.5 months (imlune monotherapy) HR=0.57 (p<0.001)

ESR1-mut+ Subgroup (PFS)

Imlunestrant (5.5 months)
Control (3.8 months)

➡ Risk of progression/death decreased by 38%.

Combination Arm (PFS)

Imlunestrant+Abemaciclib (9.4 months)
Imlunestrant mono (5.5 months)

➡ The combination provided significant superiority regardless of mutation status.

  • ORR: Imlunestrant 12% · Control 8% · Combination 27%
  • OS: Not yet mature; a favorable trend in the ESR1-mut+ group (HR=0.55).
  • Side Effects: Low-grade fatigue, diarrhea, and nausea were slightly more common with imlunestrant; diarrhea (86%), nausea (49%), and neutropenia (48%) were prominent with the combination.
  • Treatment discontinuation rate: Imlunestrant 4% · Combination 6%—remained low.

🎯 Conclusion and Clinical Commentary

The EMBER-3 studydemonstrated that, in advanced ER+/HER2− breast cancer, imlunestrant provides a clear benefit particularly in ESR1-mutated patients. In this group, the risk of progression or death decreased by 38%.

Although the monotherapy difference was limited in the overall population, combination treatment with abemaciclib significantly prolonged progression-free survival (9.4 months vs. 5.5 months), making it a promising option. This effect was observed independently of ESR1 mutation status.

💡 Clinically: Imlunestrant is a new member of the biomarker-guided treatment era. ESR1 mutation testing has become decisive in treatment decisions. Oral administration and brain penetration are additional advantages that improve adherence and treatment accessibility.

  • Biomarker selection: With FDA approval, it has become a new standard option for patients positive for an ESR1 mutation.
  • Combination approach: Use with CDK4/6 inhibitors offers potential benefit in broader patient groups.
  • Patient experience: The oral formulation and its convenience compared with intramuscular injections improve treatment adherence.

In conclusion, imlunestrant stands out as a strong candidate in endocrine-resistant advanced breast cancer, through both monotherapy and combination strategies. In the coming years, the EMBER-4 adjuvant trial and real-world data will further clarify its place in treatment algorithms.
