Speed and Convenience in Immunotherapy: The Subcutaneous Shift

Intravenous (IV) cancer infusions create major constraints on time and access . Subcutaneous (SC) immunotherapies open a new era, reducing treatment to minutes, lowering IV-access needs, and improving clinic workflow. With FDA approval on September 19, 2025 , Keytruda Qlex (pembrolizumab + berahyaluronidase alfa-pmph) can now be administered for most approved IV Keytruda indications under the skin .

✅ New Approval: Keytruda Qlex (Subcutaneous Pembrolizumab)

  • Indication scope: In adults and pediatric patients aged ≥12, all approved IV pembrolizumab solid-tumor indications.
  • Study: MK-3475A-D77 (NCT05722015), involving previously untreated metastatic NSCLC patients negative for EGFR/ALK/ROS1, randomized, multicenter, open-label.
  • Patients: 377 (2:1 randomization; SC Qlex + platinum chemotherapy vs IV pembro + platinum chemotherapy).
  • Primary aim: Pharmacokinetic comparability (Cycle-1 AUC0–6 weeks and Cycle-3 Ctrough).

Key numerical findings:

  • PK equivalence: Lower bounds of geometric mean ratios were above ≥0.8 the threshold.
  • Response rate (ORR): 45% (SC) vs. 42% (IV).
  • PFS/OS: No significant difference (descriptive analysis).
  • Administration time: Depending on dose, approximately 1–2 minutes.

Recommended dose: 395 mg pembro + 4,800 units hyaluronidase (every 3 weeks) or 790 mg pembro + 9,600 units (every 6 weeks), until progression or unacceptable toxicity.

Warnings: Immune-related adverse effects, hypersensitivity/administration reactions, complications of allogeneic stem cell transplantation, embryo-fetal toxicity (see product information).

🌐 The SC Ecosystem: Major Previously Approved Drugs

Phesgo (pertuzumab + trastuzumab + hyaluronidase-zzxf)

Fixed-dose SC combination for HER2-positive breast cancer.

  • FDA: June 29, 2020
  • Clinical rationale: Two antibodies + enzyme in one injection, without infusions.

Opdivo Qvantig (nivolumab + hyaluronidase-nvhy)

PD-1 inhibitor; SC alternative for most adult solid-tumor indications of IV Opdivo.

  • FDA: December 27, 2024
  • Administration time: ~5 minutes (CM-67T data)

Tecentriq Hybreza (atezolizumab + hyaluronidase-tqjs)

First SC PD-(L)1; for all adult IV atezolizumab indications.

  • FDA: September 12, 2024
  • Administration time: ~7 minutes (IMscin001)

Herceptin Hylecta (trastuzumab + hyaluronidase-oysk)

SC trastuzumab formulation for HER2-positive breast cancer.

  • FDA: February 28, 2019
  • Patient preference: PrefHER reported strong preference for SC.
Drug (SC) Class Administration Time Dose/Interval (Example) Key Data
Keytruda Qlex Anti-PD-1 + hyaluronidase ~1–2 min 395 mg Q3W or 790 mg Q6W ORR 45% (SC) vs 42% (IV); PK equivalent
Opdivo Qvantig Anti-PD-1 + hyaluronidase ~5 min 600–1200 mg, Q2–4W depending on regimen CM-67T: PK/efficacy consistent with IV
Tecentriq Hybreza Anti-PD-L1 + hyaluronidase ~7 min 1,875 mg/15 mL, Q3W IMscin001: SC≈IV
Herceptin Hylecta Anti-HER2 + hyaluronidase Brief administration Fixed dose, EBC/MBC HannaH/SafeHER/PrefHER

🎯 Clinical Impact: For Whom, and When?

  • Busy centers: Administration within minutes increases chair turnover and reduces chemotherapy-unit bottlenecks.
  • Patients with difficult IV access/port problems: Reduces vascular-access trauma and infusion-related delays.
  • Long-term adjuvant/preventive protocols: Supports comfort and adherence.
  • Cost–time balance: Regardless of drug cost, gains in health-system efficiency and patient time .

🔭 Looking Ahead: What Should We Expect?

  1. Wider SC immunotherapy use: In combinations, such as chemo-IO, and adjuvant protocols, an SC-first approach.
  2. Logistical innovation: Chair time and staffing standards, with discussion of home administration for selected agents/indications.
  3. Biomarkers and real-world data: Supporting SC/IV equivalence with adherence, quality-of-life, and workforce metrics.
