Microsoft’s AI MAI-DxO Is 4 Times More Successful Than Doctors at Solving the Hardest Cases

Diagnostic Delay: The Silent Crisis of Healthcare Systems
Medical diagnosis is one of the most complex and costly components of modern healthcare. Incorrect or delayed diagnoses seriously affect both patients’ quality of life and healthcare budgets. Reaching the correct diagnosis in rare or complex cases involving multiple organ systems requires coordination across several specialties. Artificial intelligence (AI)has now entered the scene with the promise of making this process both faster and more reliable .
Research published by Microsoft on 30 June 2025is one of the most ambitious initiatives in this field. Medical AI Diagnostic Orchestrator (MAI-DxO) is the name of the system that, using cases published in the New England Journal of Medicine(NEJM), correctly diagnosed 85.5% of 304 challenging cases . Experienced physicians’ average success rate on the same cases was only 20%.
🔎 Sequential Diagnosis Benchmark: Testing Real Clinical Challenges
These cases, drawn from NEJM’s “Case Records of the Massachusetts General Hospital” series:
- Involve findings across multiple systems,
- Require advanced tests and multidisciplinary assessment,
- Include step-by-step diagnostic chains mimicking real clinical workflows.
Microsoft turned these cases into an interactive platform called Sequential Diagnosis Benchmark (SD Bench) . Both clinicians and AI systems:
- Assess the initial presentation,
- Select the necessary tests and questions,
- Reach the final diagnosis step by step.
Each test has a virtual cost, allowing both accuracy and economic efficiency to be measured simultaneously.
🤖 MAI-DxO: The Power of Orchestration
MAI-DxOcombines multiple large language models, such as GPT, Claude, Gemini and DeepSeek, rather than a single model, into a ‘virtual panel of doctors’ . Each ‘agent’ takes on a different role:
- Dr Hypothesis Agent: Lists possible diagnoses.
- Dr Test Chooser Agent: Selects the most appropriate investigations.
- Dr Stewardship Agent: Reduces unnecessary testing.
- Dr Checklist Agent: Checks clinical completeness.
- Dr Challenger Agent: Challenges decisions and prompts reconsideration if necessary.
At each step, the system chooses between asking a question, ordering a test or giving a diagnosis ; decisions are weighed in terms of both accuracy and cost.

📊 Human Doctors vs MAI-DxO
| Group Compared | Correct Diagnosis Rate | Average Test Cost |
|---|---|---|
| MAI-DxO + OpenAI (o3) | 85.5% | Low |
| 21 physicians from the US and UK | 20% | High |
| Best standalone large language models | 60–70% | Medium |
MAI-DxO does more than diagnose more accurately ; it also does so at lower cost . This offers healthcare systems a dual gain in quality and sustainability.

🔮 Will AI Replace Doctors?
In the short term, no. As Microsoft also emphasizes, MAI-DxO is a research prototype without clinical approval . The aim is not to reduce the number of physicians , but to reduce errors by:
- Providing decision support to accelerate the path to the correct diagnosis,
- Automating routine tasks to ease clinicians’ workload,
- Providing a second opinion in difficult cases to minimize diagnostic errors.
For widespread use in real hospital settings, clinical validation, ethical oversight and regulatory steps must be completed.
💬 Final Thoughts
MAI-DxOreveals the threshold of ‘super-intelligence’ that artificial intelligence has reached in clinical diagnosis. With diagnostic accuracy above 85%, a cost-effective approach and transparent multi-agent architecture, the system is on its way to becoming an indispensable part of the healthcare ecosystem of the future. We are witnessing human and artificial intelligence joining forces to open the door to a new era in healthcare.
