A New Standard for BRCA-Mutated Prostate Cancer: Full FDA Approval for Rucaparib
On 17 December 2025, the FDA granted rucaparib (Rubraca) full approval for patients with metastatic castration-resistant prostate cancer (mCRPC) previously treated with AR-targeted therapy and carrying BRCA mutations . This approval converts the previous accelerated approval to regular approval and strengthens the role of PARP inhibitors in prostate cancer.
Key message: In mCRPC patients carrying BRCA1/2 mutations, rucaparib reduces progression risk by 50% compared with standard chemotherapy or a second ARPI treatment.
1) About the Approval and TRITON3
FDA approval is based on a comparison of rucaparib with physician’s choice of treatment (docetaxel or ARPI) in TRITON3. The study demonstrated a marked advantage, particularly in patients with germline or somatic BRCA1, BRCA2 mutations.
2) Patient Selection and Dosage
Patients should be selected using an FDA-approved companion diagnostic test .
• Dose: 600 mg twice daily (2 tablets of 300 mg).
• Criterion: Patients who previously received an ARPI (abiraterone/enzalutamide) but have not received chemotherapy in the mCRPC setting.
3) Efficacy: rPFS (Median, Months)
Hazard Ratio (HR): 0.50. (p < 0.0001). Overall survival (OS) was 23.2 months in the rucaparib arm and 21.2 months in the control arm, without reaching statistical significance.
4) Safety Profile (Most Common Adverse Effects)
| Adverse Effect | All Grades | Grade 3-4 |
|---|---|---|
| Fatigue (Weakness) | 61% | 7% |
| Anemia / Decreased Hb | 47% | 24% |
| Nausea | 50% | <1% |
| Neutropenia | 20%+ | 7% |
Critical Clinical Notes
ATM Mutations: Exploratory analyses showed that rucaparib was not as effective in ATM-mutated patients (HR 0.95) as in BRCA-mutated patients. Benefit comes primarily from the BRCA group.
Hematologic Monitoring: The Grade 3–4 anemia rate is high, at 24%. Monitoring complete blood counts during treatment is vital.
Use Before Chemotherapy: This approval demonstrates that rucaparib is an effective option before docetaxel as well.
DROZDOGAN Academy Commentary
The TRITON3 results show that, in mCRPC, genomic testing is now a necessity rather than a luxury. In patients with identified BRCA mutations, rucaparib can hold back disease progression for twice as long. The limited response in ATM mutations highlights the importance of a personalized approach to PARP inhibitor selection based on mutation type.
