PALB2 Is No Longer a Secondary Gene!

Most breast cancers are sporadic, but a substantial proportion involve inherited germline predisposition . BRCA1/2 are best known, yet other genes increasingly influence screening and risk reduction. PALB2 is among the most important.

PALB2 (Partner and Localizer of BRCA2)works closely with BRCA2 in DNA repair. For pathogenic variant carriers, the goal is to use age-dependent cumulative risk (penetrance), rather than a single percentage, to clarify who needs which screening or prevention strategy, and when .

Note: This is not medical advice. Personal risk and screening decisions require genetic counseling and oncology assessment.

Where and How Was the Study Presented?

The data appeared at San Antonio Breast Cancer Symposium (SABCS) 2025 in Rapid Fire Session 5 as RF5-01 . The abstract was reported in the congress J Clin Oncol supplement: J Clin Oncol. 2025;43(suppl 17):RF5-01.

Study Design

This was a risk-estimation / penetrance study rather than a drug trial. Researchers pooled germline PALB2 pathogenic variant carriers from multinational multicenter databases to estimate cumulative incidence of breast, ovarian and pancreatic cancers with increasing age.

N=5,214 carriers; median diagnosis age: 45; median follow-up: 10.2 years. Analyses reportedly used Cox and Kaplan–Meier methods.

Visual Summary: Cumulative Risk to Age 70

Ovarian

Pancreatic

Male Breast

What Do These Numbers Mean for Patients and Families?

45% breast cancer risk supports classifying PALB2 as high risk. Ovarian risk was 20% and pancreatic risk 5%.

The Most Clinically Useful Subgroup Signals

• Truncating variants may confer approximately 10% higher risk according to the presentation.
• Possible synergy in the subgroup with BRCA2 co-variants (n=150): HR 5.1.
• Ethnic differences: the Asian subgroup's breast risk was 35% .
• Family-controlled analysis reported first-degree relatives' risk reaching 60%.

PALB2's Place in Hereditary Breast Cancer

BRCA1/2 discovery in the 1990s began inherited risk management. PALB2, identified in 2007, gained visibility as panel testing expanded. Risk management now combines multiple genes + family history + age .

Are There Side Effects? The Right Framework

This genetic risk analysis does not report drug side effects. It can influence screening intensity, risk-reducing surgery and timing of medical options. The burden therefore relates to the chosen strategy's risks and psychological effects.

The presentation emphasizes validation and variant classification: high-quality testing, accurate interpretation and genetic counseling.

Practical Message

PALB2 positivity does not dictate one automatic pathway. Risk varies with age, family history, variant type, other genetic findings and personal health. Results are best addressed through personalized risk counseling .
