An Advance in Papillary Kidney Cancer: Bevacizumab–Erlotinib for Hereditary and Sporadic Disease

In this article
🧬 From Genetics to Clinical Reality: HLRCC and Papillary Kidney Cancer
Papillary renal cell carcinoma (pRCC) is the second most common kidney cancer subtype (10-15%). However, 3–5% of this broad group has a much more aggressive course associated with hereditary disorders such as Hereditary Leiomyomatosis and Renal Cell Cancer (HLRCC) syndrome.
This autosomal dominant condition involves inherited mutations in the fumarate hydratase (FH) gene. These germline mutations disrupt cellular energy metabolism and tumor suppression, causing papillary kidney tumors that arise young and can metastasize rapidly.

The characteristic clinical triad of HLRCC is:
- ✅ Painful goosebump-like skin nodules (cutaneous leiomyomas)
- ✅ Early-onset uterine fibroids, often requiring hysterectomy
- ✅ Aggressive papillary RCC, usually unilateral and capable of very early metastasis

Above, Cutaneous Leiomyomatosis: Pink/red papules and nodules varying in size and shape.
Unlike conventional type 1 pRCC, HLRCC-associated kidney tumors have type 2-like histology and may spread to lymph nodes or distant organs even when smaller than 1 cm.
FH mutation carriers have:
- A lifetime renal cell cancer risk of 15–20% .
- Diagnosis usually occurs at age 42–44, although RCC has been reported at age 11.
🔍 New Treatment Hope: Bevacizumab + Erlotinib
Until now, HLRCC-associated metastatic pRCC lacked an effective, evidence-based systemic treatment option . A new study published on 18 June 2025 in NEJMoffered striking Phase 2 findings that could change this.
The study found notable activity in both HLRCC-associated and sporadic papillary RCC with bevacizumab + erlotinib combination treatment.
Why Bevacizumab + Erlotinib?
In HLRCC-associated papillary kidney cancer, FH mutations cause intracellular fumarate accumulation, leading to HIF-1α stabilization and increased VEGF production. This promotes extensive tumor vascularization and aggressive growth. Bevacizumab targets VEGF to inhibit angiogenesis; erlotinib blocks EGFR signaling to suppress proliferation. Together, they inhibit both vessel formation and cell growth, producing synergistic antitumor activity.
🔬 Study Summary and Participants
- The study included patients with metastatic or locally advanced inoperable papillary renal cell carcinoma .
- All patients were eligible for systemic treatment for advanced disease .
- Most participants had not received prior systemic treatment, meaning the study was conducted in the first-line setting.
- Nonrandomized, single-arm, open-label Phase 2 design.
- Patients with HLRCC (germline FH mutations) and sporadic pRCC were evaluated prospectively in separate cohorts.
The two groups were:
- HLRCC-Associated Papillary RCC: 43 patients
- Sporadic Papillary RCC: 40 patients
Treatment Regimen:
- Bevacizumab: 10 mg/kg intravenously every 2 weeks
- Erlotinib: 150 mg orally 1 time daily
Primary Endpoint: Objective response rate (ORR)
Secondary Endpoints: Progression-free survival (PFS) and overall survival (OS)
📈 Findings in Numbers: Striking Responses in Hereditary Disease
✅ HLRCC-Associated Papillary RCC:
- Objective Response Rate (ORR): 72%
(31/43 patients; 95% confidence interval: 57–83%) - Median Progression-Free Survival (mPFS): 21.1 months
(95% CI: 15.6–26.6 months) - Median Overall Survival (mOS): 44.6 months
(95% CI: 32.7 months–upper limit not estimable)
These findings offer considerable hope, particularly for aggressive, treatment-resistant tumors such as HLRCC-associated disease.
⚠️ Sporadic Papillary RCC:
- Objective Response Rate (ORR): 35%
(14/40 patients; 95% CI: 22–51%) - Median PFS: 8.9 months
(95% CI: 5.5–18.3) - Median OS: 18.2 months
(95% CI: 12.6–29.3)
🚨 Treatment-Related Side Effects
Most Common Side Effects:
- Acneiform rash: 93%
- Diarrhea: 89%
- Proteinuria: 78%
Severe (Grade ≥3) Side Effects included:
- Hypertension: 34%
- Proteinuria: 17%
These effects were consistent with the previously known bevacizumab + erlotinib toxicity profile.
🔍 Why Does This Study Matter?
The study stands out in a genetically defined papillary kidney cancer subgroup as one of the first effective systemic treatment strategies . Particularly in HLRCC, median PFS of 21 months, and overall survival of 44.6 months are record levels suggesting that molecularly targeted treatments may offer hope in rare kidney cancer subtypes.
The findings also suggest some benefit from bevacizumab–erlotinib in sporadic papillary RCC, again emphasizing molecular subtyping in personalized care.
💡 Conclusion and Clinical Message
- Bevacizumab + erlotinibshowed substantial antitumor activity in both HLRCC-associated and sporadic papillary RCC.
- HLRCC and other hereditary kidney cancer syndromes are becoming settings where targeted treatment is a scientific reality.
- These findings strengthen the role of genetic testing in kidney cancer treatment and may mark a turning point for oncology strategies based on molecular diagnosis .
