Disitamab and Toripalimab Double Survival in HER2-Positive Urothelial Cancer

Disitamab and Toripalimab Double Survival in HER2-Positive Urothelial Cancer
In this article
Disitamab vedotin + toripalimab outperformed chemotherapy in HER2-expressing advanced urothelial cancer, extending overall survival to 31.5 months; HR 0.54, PFS 13.1 months.

Presented at the ESMO 2025 Presidential Symposium (LBA7)and simultaneously published in NEJM , the RC48-C016 Phase 3 trial showed that, in HER2-expressing locally advanced or metastatic urothelial cancer , first-line disitamab vedotin plus toripalimab nearly doubled overall survival compared with conventional chemotherapy.

Why Will This Study Make History?

Since the 1980s, urothelial cancer treatment has relied largely on platinum-based chemotherapy. Although these treatments extended life for many patients, toxicity and early resistance often prevented long-term control. The arrival of immunotherapy in the mid-2010s brought new hope, but only a minority achieved lasting responses. Scientists therefore began asking: Can targeted smart molecules and the immune system be activated simultaneously?

Molecular analyses in recent years have shown HER2 positivity in approximately 10–20% of urothelial cancers, identifying a biologically distinct subgroup. HER2 has therefore become a clinically meaningful target in bladder cancer as well as breast and gastric cancers. This provided a strong scientific basis for using HER2-targeted ADCs such as disitamab vedotin in this disease.

Built on this scientific foundation, RC48-C016 became a historic moment at ESMO 2025 in Berlin. More than 9,000 oncologists filled the vast hall when it was announced that Disitamab Vedotin + Toripalimab had extended median overall survival to 31.5 months, prompting resounding applause. An ADC (antibody-drug conjugate) and immunotherapy showing such powerful synergy, in the words of many experts, "opened the door to a new era in urothelial cancer treatment".

RC48-C016 is more than a successful Phase 3 trial; in oncology, it marks ADC + IO (antibody-drug conjugate + immunotherapy) becoming a clinical reality in urothelial cancer and represents a milestone. HER2 targeting combined with PD-1 blockade produced a substantial survival gain, clearly bringing the successes previously seen in lung, breast and gastric cancers to bladder cancer.

📈 Kaplan–Meier Summary

  • OS at 12 / 18 / 24 months (DV+T): 79.5% / 64.6% / 52.8%
  • OS at 12 / 18 / 24 months (Chemotherapy): 62.5% / 48.1% / 39.4%

Median follow-up: 18.2 months. The curves separate early, with the difference maintained through month 24.

Study Design and Numerical Findings

  • Phase & Code: Phase 3, RC48-C016 (LBA7)
  • Population: HER2-expressing (IHC ≥1+) locally advanced / metastatic urothelial carcinoma; first-line setting
  • Arms: DV+T (Disitamab vedotin + Toripalimab; n=243) vs platinum-based chemotherapy (n=241)
  • Co-Primary Endpoints: OS (Overall Survival) and PFS (Progression-Free Survival)
Endpoint DV+T Chemotherapy Interpretation
Median OS 31.5 months (21.7–NE) 16.9 months (14.6–21.7) HR 0.54; p < 0.0001
Median PFS 13.1 months 6.5 months HR 0.36; p < 0.0001
ORR 76.1% 50.2% + 26% absolute increase
OS at 12/18/24 months 79.5% / 64.6% / 52.8% 62.5% / 48.1% / 39.4% The advantage starts early and persists

What Does This Mean? HR 0.54 means a 46% relative reduction in death risk . In absolute terms, +14.6 months of additional OS makes a tangible contribution to patients' actual life expectancy . The ~2-fold PFS advantage may help delay symptom burden and the need to change treatment .

🛡️ Safety Profile

  • Grade ≥3 treatment-related AEs: DV+T 55.1% vs Chemotherapy 86.9% — substantially lower severe toxicity burden.
  • Peripheral neuropathy ≥3: 4.1% — an expected risk with MMAE-payload ADCs; early dose modification and treatment interruption algorithms are important.
  • Hepatic & Hematological: For liver enzyme changes and hematological toxicity, close blood count / biochemistry monitoring is recommended.

Practical Tip: Frequent assessment in patients with a neuropathy history, and prompt dose adjustment if AST/ALT rise, improve treatment continuity.

DROZDOGAN Academy Commentary

Although RC48-C016 presents groundbreaking results in HER2-positive urothelial cancer, several points need emphasis regarding generalizability and its future place in treatment.

  • Population Differences: The study was conducted entirely in a Chinese population . Pharmacogenetic differences, diet and comorbidity profiles may therefore influence treatment response. Global validation requires more ethnically diverse Phase 3 trials .
  • Comparator Arm: The control arm used platinum-based chemotherapy, which is no longer the current standard in many countries. In the US and Europe, enfortumab vedotin + pembrolizumab is now approved and reimbursed for first-line use. Establishing DV+T's true clinical superiority therefore makes direct ADC+IO comparisons critical.
  • Biomarker Heterogeneity: Interlaboratory differences in HER2 testing, including IHC scoring, ISH thresholds and specimen quality, remain a serious problem. This may directly affect accurate patient selection and reproducibility. Translation into clinical practice requires standardized, rapid and reliable HER2 testing algorithms .
  • The Sequencing Question: Many patients now receive immunotherapy in early-stage or adjuvant settings. Consequently, DV+T efficacy in patients previously treated with a checkpoint inhibitor or an ADC is not yet known. Cross-resistance and payload selection (such as MMAE) will be important determinants in the future.
  • Clinical Perception and Acceptance: Although ADC+IO combinations attract attention with dramatic efficacy, their costs, infusion times and toxicity management make real-world data crucial. Multidisciplinary management will be particularly important for neuropathy and hepatic side effects.

In Conclusion: RC48-C016 has established a new reference point for urothelial cancer treatment. Making this impact lasting worldwide will require confirmatory Phase 3 studies, biomarker standardization and clarification of treatment sequencing .
