FDA Grants Traditional First-Line Approval to the Braftovi Combination

A critical turning point in the treatment of metastatic colorectal cancer is the BRAF V600E mutation, which now has a more powerful treatment option. As of February 24, 2026, the FDA upgraded the encorafenib (Braftovi) combination to traditional approval, establishing a new standard in first-line treatment based on striking clinical data showing that it doubled patients' survival.

A New Standard in the Treatment Algorithm

BRAF V600E mutant colorectal cancers are known for their resistance to standard treatments. The FDA's traditional approval makes the 2024 accelerated approval permanent, based on the BREAKWATER Phase 3 trial's record overall survival of 30.3 months .

A Flexible Chemotherapy Backbone: The approval allows the targeted combination (Encorafenib + Cetuximab) to be combined with either mFOLFOX6 or FOLFIRI regimens, giving clinicians the opportunity to make patient-specific choices.

1. The BREAKWATER Trial and Treatment Arms

The trial evaluated three main arms and a newly added FOLFIRI cohort in previously untreated patients with BRAF V600E-mutant mCRC:

Approved Regimen

Encorafenib + Cetuximab + Chemotherapy

  • Targeted Therapy: Encorafenib + Cetuximab
  • Chemotherapy Option 1: mFOLFOX6 (Oxaliplatin)
  • Chemotherapy Option 2: FOLFIRI (Irinotecan)

Control Arm

Standard of Care

  • Standard Chemotherapy: FOLFOX / FOLFIRI / CAPOX
  • Optional: Addition of bevacizumab

2. Survival Analysis: A Historic Achievement

According to the Phase 3 BREAKWATER data, the encorafenib combination doubled survival compared with the control group.

Median Overall Survival (OS)
30.3 Months
Experimental Arm (Braftovi + mFOLFOX6)
Median Overall Survival (OS)
15.1 Months
Control Arm (Standard of Care)

Hazard Ratio (HR): 0.49 — A 51% reduction in the risk of death! (p < .0001)

3. Progression-Free Survival and Response Rates

Progression-Free Survival (PFS) - Experimental 12.8 Months
Progression-Free Survival (PFS) - Control 7.1 Months
Experimental Response Rate (ORR)
61% - 64%
Control Response Rate (ORR)
39% - 40%
DROZDOGAN Academic Note:

BRAF V600E-mutant metastatic colorectal cancer has been one of the most challenging areas in oncology, but combining encorafenib + cetuximab with chemotherapy has completely changed this picture. The overall survival of 30.3 months supporting traditional approval can be considered a “milestone” for this patient group.

Clinical Practice and Freedom of Choice:
The FDA approval covers both mFOLFOX6 and FOLFIRI backbones, allowing us to offer a personalized treatment plan according to a patient's toxicity profile (for example, choosing FOLFIRI for patients with pre-existing neuropathy). This regimen is now the undisputed new standard for first-line treatment of BRAF-mutant mCRC.

In conclusion, this triplet combination should be considered the first option for every patient with a detected BRAF V600E mutation to maximize disease control and prolong survival.
