A New Era in Weight Management: The First "Oral" GLP-1 (Wegovy Pill) Is Approved
On 22 December 2025, the US Food and Drug Administration (FDA) officially approved Novo Nordisk's Wegovy pill (oral semaglutide 25 mg) . This decision means that GLP-1 medicines, which revolutionized obesity treatment but had previously been available only as injections, can now also be taken as a daily pill. Studies demonstrated that the pill is at least as effective as the injectable form (16.6% weight loss).
Key Message: A major barrier has been removed for millions of patients unable to start GLP-1 treatment because of needle phobia or difficulties with injections. However, experts warn that the pill must be taken "on an empty stomach and in strict accordance with the instructions" to be effective.
1) Is It as Effective as the Injection? (OASIS-4 Results)
The main question was whether an oral drug would lose its effectiveness as it passed through the digestive tract. The OASIS-4 study addressed these concerns. At the end of 64 weeks:
- Wegovy Pill: Produced an average weight loss of 16.6%.
- Placebo: Weight loss was only 2.2%.
- Comparison: These results are on par with the effectiveness of the weekly Wegovy injection (around 15%), with similar results across some parameters.

Additional Benefit: The FDA approved this pill not only for weight loss but also, in adults with obesity/overweight and heart disease, to reduce the risk of heart attack and stroke (MACE) .
3) Have the Adverse Effects Changed?
No. The adverse effects are similar because the mechanism is the same. The most common complaints remain gastrointestinal (nausea, vomiting, diarrhea). Experts warn that the pill may cause somewhat more intense nausea than the injection because it reaches the stomach all at once.
Critical Warning: Instructions for Use
The biggest challenge with the Wegovy pill is the "discipline" it requires. While the injection is taken once a week and then forgotten, the pill must be taken every day under the following rules:
- First Thing in the Morning: Take it at least 30 minutes before the first food, drink, or other medicines of the day.
- The Stomach Must Be Empty: Take it on a completely empty stomach.
- Water Limit: Swallow with no more than 120 ml (half a glass) of water. More water may reduce absorption.
Pill vs. Injection: Which Is Right for You?
| Feature | Wegovy Injection | Wegovy Pill |
|---|---|---|
| Frequency | 1 time weekly | 1 time daily |
| Administration | Injection | Oral (Tablet) |
| Fasting Requirement | None | Yes (Strict Rule) |
| Efficacy | ~15% | ~16.6% |
DROZDOGAN Academy Commentary
This approval is a huge step toward improving access to obesity treatment. For many patients, "injecting themselves" was a psychological barrier; that barrier has now been removed. As clinicians, however, we face a new challenge: Patient adherence. Because food can impair the drug's absorption, strict management of the patient's morning routine will be a key factor determining treatment success.
