A New First-Line Signal in PD-L1-Positive Advanced Lung Cancer: Sacituzumab Tirumotecan + Pembrolizumab Strongly Prolonged PFS

ASCO 2026 Abstract 8506 The Lancet, 29 May 2026 OptiTROP-Lung05 PD-L1 TPS ≥1, EGFR/ALK-negative advanced NSCLC
Pembrolizumab monotherapy had become one of the main first-line treatment options in PD-L1-positive advanced non-small cell lung cancer, particularly in patients with high PD-L1 levels. However, in the broad group with PD-L1 TPS ≥1, especially in the TPS 1–49 range, the duration of disease control with immunotherapy alone often remains limited. Presented at ASCO 2026 and simultaneously published in The Lancet, the phase 3 OptiTROP-Lung05 studysignals a new threshold in this field: the combination of the TROP2-targeted antibody-drug conjugate sacituzumab tirumotecan and pembrolizumab strongly improved progression-free survival compared with pembrolizumab monotherapy.

Sacituzumab Tirumotecan Plus Pembrolizumab Shows Strong First-Line PFS Benefit in PD L1-Positive Lung Cancer, ASCO26

WHY DOES IT MATTER?

Adding an ADC to Immunotherapy: A New First-Line Rationale in Lung Cancer

Immunotherapy has fundamentally changed the treatment paradigm in advanced NSCLC, particularly in PD-L1-positive patients. While pembrolizumab monotherapy can provide long-lasting responses in patients with PD-L1 TPS ≥50, immunotherapy alone may not always provide sufficiently deep or prolonged disease control in the PD-L1 TPS 1–49 range. A frequent clinical question is therefore: should we add chemotherapy for these patients, give immunotherapy alone, or strengthen immunotherapy with a smarter and more targeted payload carrier?

OptiTROP-Lung05 offers a new answer to this question. Instead of conventional chemotherapy, sacituzumab tirumotecan, a TROP2-targeted ADC, is added to pembrolizumab. This is the idea of combining a cytotoxic payload targeting tumor cells with PD-1 blockade activating the immune system within the same treatment strategy.

Brief Concept Note: What Are NSCLC, PD-L1 TPS, and an ADC?

NSCLCstands for non-small cell lung cancer and is the most common subgroup of lung cancers. PD-L1 TPSis the tumor proportion score showing the percentage of tumor cells staining for PD-L1; it is used when selecting immunotherapy. ADCstands for antibody-drug conjugate; it is based on the principle of an antibody binding to a specific target on a cancer cell and carrying a potent chemotherapy-like payload into the cell.

How Does Sacituzumab Tirumotecan Work?

Sacituzumab tirumotecan is a TROP2-targeted antibody-drug conjugate. TROP2 is a protein expressed on the tumor cell surface in many epithelial cancers and in NSCLC. The antibody component binds to TROP2; once transported into the cell, the drug payload acts as a topoisomerase I inhibitor, causing DNA damage with the aim of killing the tumor cell.

The theoretical advantage of this approach is to deliver the cytotoxic effect more selectively to tumor cells and potentially create a “bystander” effect that also affects surrounding tumor cells. Pembrolizumab, as a PD-1 inhibitor, releases the T-cell response. Combining these two effects may strengthen both tumor shrinkage and the immune response.

HOW WAS THE STUDY DESIGNED?

A Phase 3 Study of 413 PD-L1-Positive, EGFR/ALK-Negative Patients

OptiTROP-Lung05 is a randomized, open-label, multicenter phase 3 study. It enrolled patients with stage IIIB/IIIC or stage IV NSCLC who had received no previous systemic antitumor therapy for locally advanced or metastatic disease, had PD-L1 TPS ≥1, no EGFR or ALK alterations, and an ECOG performance score of 0–1.

Patients were assigned to two arms in a 1:1 ratio. In the combination arm, sacituzumab tirumotecan 4 mg/kg was given every 2 weeks and pembrolizumab 400 mg every 6 weeks. In the control arm, pembrolizumab 400 mg was administered every 6 weeks as monotherapy. The primary endpoint was progression-free survival according to blinded independent central review.

Combination arm

Sacituzumab tirumotecan 4 mg/kg every two weeks + pembrolizumab 400 mg every six weeks.

Control arm

Patient selection

PD-L1 TPS ≥1, EGFR/ALK-negative, locally advanced or metastatic NSCLC; ECOG 0–1.

Brief Concept Note: What Do Open-Label Phase 3 and BICR Mean?

Open-labelis a study design in which the patient and physician know which treatment is being given. Phase 3is the advanced stage of clinical research in which a new treatment is compared with standard or control treatment in a large patient population. BICRstands for blinded independent central review, meaning centralized evaluation of imaging results by independent experts who do not know the treatment arm.

MAIN EFFICACY RESULTS

A Very Strong Difference in PFS: HR 0,35

At the data cutoff of 29 September 2025, median follow-up was 10,5 months. According to BICR, median PFS had not yet been reached in the sacituzumab tirumotecan + pembrolizumab arm; in the pembrolizumab monotherapy arm it was 5,7 months . The hazard ratio was reported as 0,35 , meaning an approximately 65% relative reduction in the risk of progression or death. The P value was <0,0001 .

Brief Concept Note: How Should PFS and the Hazard Ratio Be Interpreted?

PFSmeans progression-free survival; it indicates the period during which the disease remains controlled without progression or death. The hazard ratiocompares the risk of an event developing over time in two treatment arms. HR 0,35 shows that the risk of progression or death was markedly lower in the combination arm than in the control arm.

Clinical Meaning of the PFS Result

  • The primary endpoint was met: Sac-TMT + pembrolizumab improved PFS compared with pembrolizumab monotherapy in a statistically and clinically meaningful manner.
  • The risk reduction is large: HR 0,35 is a particularly strong PFS signal in first-line advanced NSCLC.
  • 12-month PFS more than doubled: 12-month PFS was 62,4% in the combination arm versus 29,0% in the control arm.
  • Consistent across subgroups: The PFS benefit appeared consistent in the PD-L1 TPS 1–49, TPS ≥50, squamous, and nonsquamous subgroups.

The Difference in PD-L1 Subgroups Is Particularly Striking in the TPS 1–49 Group

In the PD-L1 TPS ≥50 group, the HR favoring the combination was 0,47 . In the PD-L1 TPS 1–49 group, the HR was reported as 0,28 . This finding is clinically important because patients with TPS 1–49 are the group in which pembrolizumab alone is most debated. Adding an ADC in this group may fill the biological gap where immunotherapy monotherapy can prove insufficient.

Brief Concept Note: Why Is PD-L1 TPS 1–49 a Distinct Group?

Pembrolizumab monotherapy is a strong standard option in patients with PD-L1 TPS ≥50. However, in the TPS 1–49 group, response to immunotherapy alone may be more limited, and chemotherapy-immunotherapy is preferred for many patients. The HR of 0,28 in the TPS 1–49 group in OptiTROP-Lung05 suggests that an ADC + immunotherapy strategy may be particularly important in this intermediate PD-L1 group.

Response Rate of 70,2%: More Frequent and Deeper Tumor Shrinkage

The objective response rate in the combination arm was %70,2, compared with %42,0 in the pembrolizumab monotherapy arm. The absolute difference was reported as approximately 28,3 percentage points . In addition, deep response, meaning at least 50% shrinkage in the sum of target lesion diameters, occurred in %49,0in the combination arm versus %25,9 in the pembrolizumab arm.

Brief Concept Note: What Are ORR and Deep Response?

ORRis the objective response rate; it indicates the proportion of patients whose tumors shrink measurably. Deep responserefers to a greater reduction in tumor burden. In this study, deep response was defined as at least a 50% reduction in the sum of target lesion diameters.

Overall Survival Is Not Yet Mature, but the Early Signal Favors the Combination

Overall survival data were not yet mature at the interim analysis and were assessed descriptively by the investigators. There were 33 deaths (15,9%) in the combination arm and 54 deaths (26,3%) in the pembrolizumab arm. Median OS had not yet been reached in the combination arm; it was reported as 14,5 months in the pembrolizumab arm. The HR was 0,55 . The 12-month OS rate was %80,4in the combination arm versus %68,9 in the pembrolizumab arm.

Truecheck Note: OS Data Are Not Mature

Although the OS trend appears to favor the combination, this is an interim PFS analysis, and overall survival data are not yet mature. The main evidence at present therefore concerns PFS and response rate. Longer follow-up will show whether OS ultimately translates into a meaningful and clinically strong benefit.

SAFETY

Efficacy Increases, and So Does the Burden of Adverse Effects

Sac-TMT + pembrolizumab provided greater efficacy, but the burden of adverse effects was higher than with pembrolizumab monotherapy. Grade 3 or higher treatment-emergent adverse events occurred in %55,3in the combination arm versus %31,4 in the pembrolizumab arm. Serious adverse event rates were reported as %38,9 and %28,9 , respectively.

The most frequently reported adverse effects in the combination arm were hematologic and mucosal toxicities: anemia, decreased neutrophils, decreased leukocytes, alopecia, and stomatitis were prominent. Ocular surface toxicity and infusion reactions were also among adverse effects of special interest for Sac-TMT.

Notable Adverse Effects

  • Anemia: In the combination arm, any grade 87,5%, grade ≥3 9,1%.
  • Decreased neutrophils: Any grade 44,7%, grade ≥3 17,3%.
  • Stomatitis: Any grade 40,4%, grade ≥3 5,3%.
  • Alopecia: Any grade 65,9%; grade ≥3 was not reported.
  • Ocular surface toxicity: 14,4% as an adverse effect of special interest for Sac-TMT; grade ≥3 was not reported.
  • Pneumonitis: Among pembrolizumab immune-related adverse effects in the combination arm, it was reported in 12,5%; grade ≥3 in 2,9%.

Brief Concept Note: What Are TEAE and Stomatitis?

TEAEstands for treatment-emergent adverse event, meaning an adverse event developing during treatment; whether it is definitely related to treatment is assessed separately. Stomatitisis inflammation of the oral mucosa that can cause pain, sores, burning, and difficulty eating. It is important for quality of life and continuity of dosing in ADC treatment.

Clinical Balance

This combination carries a higher burden of adverse effects than pembrolizumab monotherapy. Clinical decisions should therefore be based not only on the PFS gain, but also on the patient's performance status, hematologic reserve, risk of mucositis, expectations for quality of life, access to treatment, and capacity to manage toxicity.

HISTORICAL CONTEXT

From Pembrolizumab Monotherapy to the Era of ADC + Immunotherapy?

Pembrolizumab monotherapy became established in clinical practice for PD-L1-positive advanced NSCLC through studies such as KEYNOTE-024 and KEYNOTE-042. KEYNOTE-024 demonstrated the superiority of pembrolizumab monotherapy over chemotherapy in patients with PD-L1 TPS ≥50. KEYNOTE-042 opened the door to the PD-L1 TPS ≥1 patient group; however, the benefit is known to be stronger particularly in the TPS ≥50 group, with monotherapy decisions more controversial in the TPS 1–49 group.

Meanwhile, pembrolizumab + chemotherapy became standard in both nonsquamous and squamous advanced NSCLC through KEYNOTE-189 and KEYNOTE-407. Adding chemotherapy, however, means greater toxicity, hair loss, myelosuppression, and treatment burden. An ADC + immunotherapy strategy offers a new possibility in this equation: a tumor-targeted cytotoxic payload instead of conventional chemotherapy.

KEYNOTE-024 PD-L1 TPS ≥50, EGFR/ALK-negative advanced NSCLC Pembrolizumab monotherapy vs chemotherapy Pembrolizumab monotherapy became a strong standard in the high-PD-L1 group. OptiTROP tests whether adding an ADC can take this standard toward stronger disease control.
KEYNOTE-042 PD-L1 TPS ≥1, EGFR/ALK-negative advanced NSCLC Pembrolizumab monotherapy vs chemotherapy The monotherapy option expanded for PD-L1 TPS ≥1, but benefit is more pronounced at TPS ≥50. The HR of 0,28 in the TPS 1–49 group in OptiTROP is particularly striking.
KEYNOTE-189 / 407 Nonsquamous and squamous advanced NSCLC Pembrolizumab + platinum-based chemotherapy A chemotherapy-immunotherapy standard was established regardless of PD-L1 level. OptiTROP offers a new direction that strengthens immunotherapy with an ADC instead of conventional chemotherapy.

This Table Is Not a Direct Head-to-Head Comparison

Different studies differ in patient populations, PD-L1 distributions, histology proportions, follow-up durations, treatment arms, and endpoints. OptiTROP-Lung05 results should therefore not be read as a claim of direct superiority over the KEYNOTE studies. The appropriate interpretation is that OptiTROP demonstrated strong PFS superiority over pembrolizumab monotherapy within the same study.

WHY IS THIS SO STRIKING IN THE TROP2 ADC FIELD?

TROP2 ADCs Have a Challenging History in Lung Cancer

TROP2-targeted ADCs attract considerable interest in many solid tumors. Their path in lung cancer, however, has not been straightforward. Some TROP2 ADC studies did not fully meet expectations; the clinical weight of PFS and OS gains was particularly debated in unselected advanced NSCLC populations. The positive phase 3 result of OptiTROP-Lung05 may therefore be an important signal of renewed confidence not only for sacituzumab tirumotecan but also for the TROP2 ADC class.

Pharmaceutical features that distinguish Sac-TMT include TROP2 targeting, a belotecan-derived topoisomerase I inhibitor payload, a pH-sensitive linker, and a high drug-to-antibody ratio. What matters most clinically, however, is whether these technological differences translate into patient outcomes. OptiTROP-Lung05 shows that this translation is strong at the PFS level.

Brief Concept Note: What Are TROP2 and the Bystander Effect?

TROP2is a protein present on the surface of many cancer cells that may be associated with tumor growth. ADCs target this protein to carry a cytotoxic payload to tumor cells. The bystander effectmeans that the drug carried by an ADC can affect not only the target cell but also nearby tumor cells. This may be important when target expression is heterogeneous within a tumor.

REGULATORY AND PRACTICAL IMPLICATIONS

Priority Review Has Begun in China; OS and Approval Processes Will Be Awaited for a Global Standard

According to Kelun-Biotech, a supplemental new drug application for sacituzumab tirumotecan + pembrolizumab was accepted by China's National Medical Products Administration and granted priority review. The application concerns the first-line treatment indication in locally advanced or metastatic NSCLC with PD-L1 TPS ≥1, negative EGFR, and negative ALK.

Nevertheless, several factors will determine the impact on global clinical practice: maturation of OS data, validation of the safety profile in different populations, authorization processes in different regions, the need for direct rather than indirect comparisons with pembrolizumab + chemotherapy or other chemo-immunotherapy standards, and cost-effectiveness assessments.

Questions Awaiting Answers Before Entry into Clinical Practice

  • Will OS mature? The early OS trend favors the combination, but the final survival result should be awaited.
  • What will its place be relative to chemo-IO? The combination is strong against pembrolizumab monotherapy, but its clinical position relative to chemotherapy-immunotherapy standards needs clarification.
  • Does the TPS 1–49 group have a particular advantage? HR 0,28 is particularly striking in this group; validation and a practical algorithm are important.
  • Can toxicity be managed? Hematologic toxicity, stomatitis, ocular surface toxicity, and immune-related adverse effects should be monitored together.
  • How will patients be selected? Can TROP2 expression, PD-L1 level, histology, and clinical risk be assessed together?
DROZDOGAN ACADEMIC COMMENTARY

Adding a Targeted Cytotoxic Payload to Immunotherapy May Open a New Chapter in First-Line NSCLC

Academic Assessment

OptiTROP-Lung05 provides highly important phase 3 data showing that adding a TROP2-targeted ADC to pembrolizumab monotherapy can markedly improve PFS in PD-L1-positive advanced NSCLC.

A strong PFS signal such as HR 0,28, particularly in the PD-L1 TPS 1–49 group, suggests that an ADC + immunotherapy combination may establish a new treatment rationale in a broad patient population for whom immunotherapy alone can be insufficient.

Nevertheless, because OS data are not yet mature and grade ≥3 adverse effects increase, the place of this approach should be defined carefully. In the future, the combination may become a new first-line strategy that serves as an alternative to, or competes with, conventional chemotherapy-immunotherapy; however, long-term survival, safety, and patient selection data will need to mature.

HOW SHOULD WE DISCUSS THIS WITH PATIENTS?

Stronger Disease Control, Closer Monitoring of Adverse Effects

Patient-Friendly Explanation

“In this study, a new targeted drug that delivers a potent medicine into cancer cells was added to the immunotherapy called pembrolizumab, which is used in PD-L1-positive advanced lung cancer. Compared with pembrolizumab alone, the combination markedly delayed disease progression and produced more tumor shrinkage. However, adverse effects were also more frequent; anemia, low white blood cell counts, mouth sores, and some immune-related adverse effects in particular should be closely monitored. Longer follow-up is needed to determine its effect on life expectancy.”

CONCLUSION

OptiTROP-Lung05 May Begin an Era of ADC + Immunotherapy in First-Line Lung Cancer

OptiTROP-Lung05 showed that sacituzumab tirumotecan + pembrolizumab provided strong PFS superiority over pembrolizumab monotherapy in PD-L1 TPS ≥1, EGFR/ALK-negative advanced NSCLC. Median PFS had not yet been reached in the combination arm, whereas it was 5,7 months in the pembrolizumab arm; HR 0,35 and P<0,0001 demonstrate that the difference is statistically very strong.

The response rate was also markedly higher: ORR was 70,2% versus 42,0%. Deep response rates were reported as 49,0% versus 25,9%. Although OS data are not yet mature, the early trend appears to favor the combination. Conversely, because grade ≥3 adverse effects were higher in the combination arm, the efficacy gain and burden of toxicity should be assessed together.

This study brings a new class combination into consideration for first-line treatment in advanced NSCLC: immunotherapy + a TROP2-targeted ADC. If the favorable signal is maintained when OS data mature and safety remains manageable, sacituzumab tirumotecan + pembrolizumab may become a new treatment paradigm, particularly in PD-L1-positive patients in whom adequate control with immunotherapy alone is difficult.
