What is ribociclib (Valamor)?

Last updated: October 2026

Ribociclib is an oral CDK4/6 inhibitor used with hormone therapy for hormone receptor-positive, HER2-negative breast cancer. It is among the drugs shown to prolong survival in metastatic disease; it has also been approved in the US and Europe for high-risk early breast cancer since 2024. In Türkiye it is sold as Valamor, and SGK reimburses it for metastatic disease with a medical board report.

Quick Answer · Physician Summary
  • Who it is used for: Hormone receptor-positive, HER2-negative breast cancer, in combination with an aromatase inhibitor or fulvestrant. Ribociclib is never given alone.
  • Evidence: In three large phase 3 trials (MONALEESA-2, -3, -7), it prolonged median overall survival in metastatic disease by 10-12 months. In early disease (NATALEE), it reduced the risk of recurrence at 5 years by 28%.
  • How it is taken: Once daily for the first 21 days of a 28-day cycle, with or without food; 600 mg for metastatic disease and 400 mg for early disease (for 3 years).
  • Monitoring is essential: Heart rhythm (ECG), liver tests, and blood counts are checked regularly, particularly during the first 6 months; it is not used with tamoxifen.

Why does this matter?

About two-thirds of breast cancers are the hormone receptor-positive, HER2-negative subtype. For many years, hormone-suppressing drugs alone were the standard treatment for this disease. Adding CDK4/6 inhibitors to hormone therapy nearly doubled the time metastatic disease remained under control without chemotherapy, and ribociclib became a drug in this class shown to prolong overall survival in three separate phase 3 trials.

Although ribociclib is taken orally and is generally well tolerated, its effects on heart rhythm, the liver, and blood counts require regular monitoring. In Türkiye, reimbursement conditions may be narrower than the licensed indication, and early-stage use does not yet appear to be covered by licensing or reimbursement, making these details important for patients' access to treatment.

What is ribociclib, and how does it work?

Ribociclib is an oral small-molecule CDK4/6 inhibitor. By inhibiting the cyclin D-CDK4/6 complex that initiates cell division, it allows the Rb protein to continue acting as a "brake," thereby arresting cancer cells in the G1 phase. It is used with an aromatase inhibitor or fulvestrant for hormone receptor-positive, HER2-negative breast cancer.

In hormone-positive breast cancer cells, estrogen signaling increases cyclin D levels; cyclin D activates the CDK4 and CDK6 enzymes, which phosphorylate the Rb protein and allow the cell to enter the DNA-copying phase (S phase). While hormone therapy reduces the signal at its source, ribociclib blocks an enzyme at the next step. This is why the two drugs are more effective together. We explain the role of this drug class in breast cancer treatment in our article on CDK4/6 inhibitors.

Figure 1 · How does ribociclib work?

The roles of hormone therapy and ribociclib at the starting point of cell division.

How to read this: The left column shows how, without the drug, estrogen signaling acts through cyclin D and CDK4/6 to release the Rb brake and push the cell to divide. The right column shows how ribociclib inhibits CDK4/6 and arrests the cell in the G1 phase.

Some tumors develop resistance to the drug over time. Known mechanisms include loss of the Rb gene (RB1), increased CDK6 levels, and continued division signaling through the cyclin E-CDK2 pathway. We discuss which treatments can be selected after a CDK4/6 inhibitor in our EVERA article.

When was ribociclib approved, and on what evidence?

Date Approval
13 March 2017 (FDA) First-line treatment with an aromatase inhibitor for postmenopausal women with metastatic disease (MONALEESA-2). European approval: 24 August 2017.
18 July 2018 (FDA) With an aromatase inhibitor for premenopausal and perimenopausal women; with fulvestrant for postmenopausal women as first-line treatment or following previous hormone therapy.
December 2021 (FDA) Male patients were also added to the indication wording.
17 September 2024 (FDA) Postoperative (adjuvant) use for 3 years with an aromatase inhibitor in stage 2-3 early breast cancer at high risk of recurrence (NATALEE). European approval: 27 November 2024.

How to interpret this: The scale is 0-70 months. In all three trials, adding ribociclib reduced the risk of death by 24-27% (HR 0.73-0.76). In MONALEESA-2, the time to disease progression also increased from 16.0 to 25.3 months.

In the phase 2 RIGHT Choice trial in premenopausal women with rapidly progressing disease, ribociclib plus hormone therapy prolonged the time to disease progression compared with combination chemotherapy (21.8 versus 12.8 months). We explain its use in metastatic disease in detail in our articles on treatment of metastatic breast cancer with Valamor and ribociclib and fulvestrant.

In early breast cancer: NATALEE

The NATALEE trial enrolled patients with stage 2-3 disease at high risk even without lymph node involvement (grade 3 tumors, or grade 2 tumors with Ki-67 of 20% or higher or high risk on genomic testing). Ribociclib at 400 mg was added to an aromatase inhibitor for 3 years. In the 5-year results announced in 2025, invasive disease-free survival was 85.5% versus 81.0%; the absolute benefit was 4.5 points, and relative risk reduction was 28% (HR 0.72). There is a favorable trend in overall survival, but it is not yet statistically significant (HR 0.80). We discuss these findings in our article comparing it with abemaciclib.

How is ribociclib used?

Metastatic disease Early disease (adjuvant)
Daily dose 600 mg (3 tablets) 400 mg (2 tablets)
Schedule 21 days on treatment, 7 days off 21 days on treatment, 7 days off
Duration For as long as it is effective and tolerated 3 years
Dose reduction 600 → 400 → 200 mg 400 → 200 mg
  • Taking the medicine: It can be taken once daily, preferably at the same time each day, with or without food. Swallow the tablet whole; do not chew, crush, or split it.
  • Missed dose or vomiting: Do not take an extra dose that day; take the next dose at the usual time.
  • Medication used with it: An aromatase inhibitor (letrozole or anastrozole) or fulvestrant. An LHRH agonist to suppress ovarian function is also added in premenopausal women. It is not used with tamoxifen, because the effect on heart rhythm is increased.
  • Pregnancy and breastfeeding: Effective contraception should be used, and breastfeeding avoided, during treatment and for at least 3 weeks after the last dose.

What are the side effects of ribociclib?

The main side effects reported in the US product information at 600 mg for metastatic disease (MONALEESA-2) and 400 mg for early disease (NATALEE) are:

Side effect 600 mg, metastatic 400 mg, early disease
Low neutrophil count (laboratory) 93% (severe 60%) 94% (severe 45%)
Elevated ALT (laboratory) 46% (severe 10%) 45% (severe 8%)
Nausea 52% 23%
Fatigue 37% 22%
Diarrhea 35% 15%
Hair thinning or loss 33% 15%
Headache 22% 23%
Dose reduction 45% 23%
Discontinuation due to side effects 7% 20%

Although a low neutrophil count is common, it leads to infection with fever less often than chemotherapy-related neutropenia; it is usually managed by interrupting treatment or reducing the dose. The higher discontinuation rate in early disease is also related to a lower tolerance for side effects among people without active disease.

Serious side effects to watch for
  • QT prolongation affecting heart rhythm: At 600 mg, QTcF exceeded 500 ms in 1.4% of patients; at 400 mg, the rate was 0.3%. Seek medical attention immediately for palpitations, dizziness, or fainting.
  • Liver injury: Yellowing of the skin or eyes, dark urine, or pain in the upper right abdomen.
  • Lung inflammation (pneumonitis): In about 1.5% of patients; new cough or shortness of breath.
  • Severe skin reactions: Rare; widespread rash, fluid-filled blisters, or skin peeling with fever.

What monitoring is performed during treatment?

Figure 2 · Monitoring schedule

How to read this: Checks are most frequent in the first two cycles; they are repeated at the start of each cycle in cycles 3-6 and as needed afterward.

According to the current US product information, an ECG is performed before treatment and around day 14 of the first cycle, then repeated as clinically needed; treatment is started only if QTcF is below 450 ms. Your oncologist determines how often imaging is performed to monitor the disease.

Which medicines and foods interact with ribociclib?

Interacting substance Effect What is done?
Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin) Ribociclib levels increase Avoid; if essential, reduce the dose to 400 in metastatic disease and 200 mg in the adjuvant setting
Strong CYP3A4 inducers (e.g., rifampicin, carbamazepine, phenytoin, St. John's wort) Ribociclib levels fall, reducing its effect Do not use together
QT-prolonging drugs (e.g., some antiarrhythmics, antibiotics, and antidepressants) Risk of abnormal heart rhythm increases Avoid; consult a pharmacist and doctor before starting a new medicine
Tamoxifen QT prolongation increases Do not use together
Grapefruit and its juice Ribociclib levels may increase Do not consume; the same recommendation is made for pomegranate and its juice

Licensing and SGK reimbursement in Türkiye

The situation in Türkiye · September 2026
  • Product: Valamor 200 mg, 63 film-coated tablets (license holder: Farmanova Sağlık Hizmetleri). There is no other ribociclib product in Türkiye. Retail price: 73,519.63 TL (from 1 April 2026). One box contains one cycle of treatment at the metastatic dose (21 days × 3 tablets).
  • Licensed indication (SmPC 4.1): Estrogen receptor positivity of at least 10%, HER2-negative metastatic breast cancer in premenopausal, perimenopausal, or postmenopausal women. With an aromatase inhibitor, in patients who have not previously received endocrine therapy for metastatic disease; with fulvestrant, in patients who have not previously used fulvestrant and whose disease has progressed during or after an aromatase inhibitor. An LHRH agonist is added in premenopausal women.
  • Reimbursement (SUT 4.2.14.C): Reimbursed with a medical board report valid for up to 6 months, with at least one medical oncology specialist on the board. With letrozole, for patients who have never received endocrine therapy or who relapse 12 months after completing adjuvant treatment; with fulvestrant, for progression during an aromatase inhibitor (after at least 6 months of use), or relapse during adjuvant aromatase inhibitor treatment or within 12 months of its completion. When the report is renewed, it must state that there is no disease progression. Because the 2020 SUT text refers to postmenopausal patients, reimbursement conditions for premenopausal patients should be confirmed in the current text.
  • Early-stage (adjuvant) use: This indication is absent from the SmPCs we reviewed, and the SUT contains no provision for adjuvant reimbursement. Off-label use approval from TİTCK is required for this use; approval does not mean SGK will reimburse it.

We described the history of the reimbursement decision in our article on the addition of palbociclib and ribociclib to the SGK list.

Frequently asked questions

Is ribociclib chemotherapy?
No. Ribociclib is a targeted drug that acts on CDK4/6 enzymes regulating cell division and is used with hormone therapy. Hair loss is milder and infection with fever less common than with chemotherapy, but regular blood tests and ECG checks are required.
Should Valamor be taken on an empty stomach or with food?
Either is fine. Take it once daily, preferably at the same time each day, swallowing the tablet whole. Do not repeat a missed or vomited dose that day; resume at the usual time the next day.
Does SGK reimburse ribociclib for early breast cancer?
As of September 2026, the SmPC and SUT texts we reviewed do not include early-stage (adjuvant) use. Off-label approval from TİTCK is required, and reimbursement is assessed separately. Confirm the current situation with your oncologist and hospital pharmacy.

DROZDOGAN Academy Commentary

Ribociclib is a drug shown to prolong survival in hormone-positive metastatic breast cancer in three separate phase 3 trials. Its role in early disease is still being debated.

⚡ Why it matters

It has enabled many patients with metastatic disease to be managed for years with oral treatment without needing chemotherapy.

✅ Current evidence

Overall survival improved in MONALEESA-2, -3, and -7. In NATALEE, the risk of recurrence at 5 years fell by 28%, and the difference was maintained over time.

⚠️ Limitations

In early disease, the absolute benefit is 4.5 points, and overall survival data are not yet mature. One in five patients with early disease stopped the drug because of side effects. Adjuvant use does not appear to be covered by licensing or reimbursement in Türkiye.

🩺 What to do in practice
  1. During the first 6 months, keep up with blood tests and ECG checks.
  2. Ask about every new medicine in terms of interactions.
  3. Grapefruit and St. John's wort should be avoided.
  4. For palpitations, yellowing of the skin or eyes, or shortness of breath, promptly seek medical attention.
❓ Open questions

The effect on overall survival in early disease, which patients without lymph node involvement benefit most, and which CDK4/6 inhibitor is better suited to which patient remain unclear. We discuss the criticisms and responses in a separate article.

Our brief judgment: In hormone-positive metastatic breast cancer, ribociclib with hormone therapy is a standard option supported by strong evidence; regular monitoring and attention to drug interactions are the keys to safe use.
