A New Era in CLL and SLL Treatment: The Acalabrutinib and Venetoclax Combination
The FDA has approved a new chemotherapy-free, fixed-duration treatment regimen for patients with chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL).
Scope of Approval and the New Standard
On 7 February 2026, the FDA approved the combination of acalabrutinib (Calquence) and venetoclax (Venclexta) for patients with previously untreated chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).
- A Chemotherapy-Free Option: The synergistic use of two targeted agents.
- Fixed-Duration Treatment: Treatment completed according to a defined schedule rather than continued indefinitely.
- High Efficacy: Superior progression-free survival compared with standard chemoimmunotherapy.
The AMPLIFY Study: Clinical Data
This approval is based on the phase III AMPLIFY study involving 520 patients. Patients were randomized to acalabrutinib + venetoclax or standard chemoimmunotherapy (FCR or Obinutuzumab + Chlorambucil).

Recommended Treatment Schedule
Treatment begins with acalabrutinib and then continues with the addition of venetoclax. This approach aims to minimize the risk of tumor lysis syndrome (TLS).
| Drug | Administration | Timing |
|---|---|---|
| Acalabrutinib | 100 mg twice daily | From cycle 1 |
| Venetoclax | Weekly dose escalation (Ramp-up) | Added in cycle 4 |
| Total Duration | Usually 12 cycles (Approximately 1 year) | |
The shift from "indefinite treatment" to "fixed-duration treatment" in CLL is revolutionary for patients' quality of life. The combination of Acalabrutinib and Venetoclax produces deep molecular responses by targeting two different biological pathways (BTK and BCL-2) simultaneously.
This approval offers the possibility of long-term remission without the need for chemotherapy, especially for patients with high-risk genetic features.
