Carboplatin, Gemcitabine and Durvalumab Treatment Protocol for Bladder Cancer

In this article
Summary
Durvalumab + gemcitabine + carboplatin is an option considered in clinical practice, particularly for cisplatin-ineligible patients with urothelial bladder cancer.
Drugs and Mechanism of Action
- Gemcitabine: Antimetabolite; inhibits DNA synthesis.
- Carboplatin: Platinum compound; inhibits replication through DNA crosslinks (less nephrotoxicity/ototoxicity than cisplatin, with a tendency toward more thrombocytopenia ).
- Durvalumab (Imfinzi): Enhances T-cell activation through PD-L1 blockade, strengthening the immune-mediated antitumor response.
The combination aims for rapid tumor burden reduction through chemotherapy and lasting immune control through immunotherapy.
Indication and Regulatory Status
- FDA approval (perioperative): only cisplatin + gemcitabine + durvalumab (neoadjuvant) → surgery → adjuvant durvalumab; March 28, 2025was the approval date. The carboplatin version is not approved.
- Guideline framework (2025): The “preferred” perioperative approach is cisplatin-based. Carboplatin use is considered individually when cisplatin is contraindicated.
- Evidence note: Durvalumab + gemcitabine with carboplatin arms have been studied across the urothelial cancer spectrum in phase II or exploratory trials, including upper tract cohorts.
Patient Selection (Practical)
- Cisplatin-ineligible urothelial bladder cancer (e.g., Ccr <60 mL/min, auditory neuropathy, severe comorbidities).
- Stage context: Perioperative assessment in muscle-invasive bladder cancer should prioritize a clinical trial ; in metastatic/migrating disease, individualize with institutional board approval.
- Performance: ECOG 0–2 (according to center protocol).
Dosing and Schedule
In the Neoadjuvant or Metastatic Setting
| Drug | Dose | Day | Administration |
|---|---|---|---|
| Durvalumab | 1500 mg IV | Day 1 | ~1-hour infusion |
| Carboplatin | AUC 4.5–5 (Calvert) | Day 1 | ~30–60 min infusion |
| Gemcitabine | 1000 mg/m² IV | Days 1 and 8 | ~30 min infusion |
Cycle: 21 days = 1 cycle | Total: 4 cycles (neoadjuvant example). In metastatic disease, 4–6 cycles according to response/tolerance.
Surgery (Perioperative scenario)
Radical cystectomy (pelvic LND in appropriate cases).
Adjuvant Phase
- Durvalumab: 1500 mg IV every 4 weeks; a total of 8 cycles (≤12 months).
Infusion Times (Estimated)
- Day 1: Up to ~3–4 hours with premedication + fluids.
- Day 8: Usually ~30 min (gemcitabine alone).
- Actual times vary by center and tolerance.
Note: Doses are provided as “institutional examples”; they must be finalized according to the center’s protocol and board approval.

SGK / Status in Türkiye
- Gemcitabine & Carboplatin:Reimbursed by SGK.
- Durvalumab (Imfinzi):Licensed in Türkiye; however, the perioperative muscle-invasive bladder cancer indication is not yet reimbursed by SGK.
- Durvalumab (Imfinzi) price:74,425.28 TL for a 500 mg vial and 17,856.38 TL for a 120 mg vial.
Special Situations Requiring Attention
Hematological Toxicity
- Thrombocytopenia (more pronounced with carboplatin), neutropenia, anemia.
- Complete blood count before each cycle; G-CSF and dose modification if needed.
Kidney / Liver
- Carboplatin dosing is based on GFR; renal function should be monitored regularly.
- Adjust Gemcitabine doses for elevated hepatic enzymes.
Immune-Mediated Events (Durvalumab)
- Thyroid dysfunction, hepatitis, pneumonitis, colitis, dermatitis → guideline-based steroid management.
Nausea/Emetogenicity
- Carboplatin is moderately emetogenic; plan appropriate antiemetic prophylaxis.
Side Effects (Expected)
- Nausea, vomiting; loss of appetite, fatigue.
- Hematological: thrombocytopenia (>carboplatin), neutropenia, anemia; fever.
- Laboratory: creatinine/GFR changes; liver enzymes.
- Immune-mediated: thyroid disorders, skin rash/itching, rarely pneumonitis.
- GI/urinary complaints such as urinary tract infection, diarrhea/constipation.
Evidence and Context
- FDA (28 Mar 2025): Perioperative cisplatin + gemcitabine + durvalumab (neoadjuvant) → surgery → adjuvant durvalumab is approved.
- NCCN 2025: The preferred perioperative approach is “cisplatin-based.”
- Carboplatin-arm data: Durvalumab + gemcitabine with carboplatin has been investigated in phase II/exploratory trials (including upper tract cohorts); institution-based assessment is recommended.
