Erdafitinib (Balversa) is a pan-FGFR tyrosine kinase inhibitor that selectively blocks fibroblast growth factor receptors (FGFR1-4). It specifically stops the uncontrolled proliferation, survival, and angiogenesis signals caused by FGFR gene alterations in cancer cells.
This monograph examines erdafitinib's mechanism of action, evolving FDA approvals, clinical findings from the phase 3 THOR study, serum phosphate-guided dose titration, monitoring for eye toxicity/hyperphosphatemia, and current licensing, SGK reimbursement, and detailed price analysis in Turkey. These topics are covered in the following sections.
Approximately 20% of urothelial carcinomas (bladder cancers) and 30-37% of upper urinary tract tumors have an FGFR3 mutation or fusion. Continuous activation of FGFR receptors causes excessive tumor growth, invasion, and resistance to chemotherapy.
Balversa (erdafitinib) blocks this overactive signaling by occupying the ATP-binding site of FGFR1, FGFR2, FGFR3, and FGFR4 tyrosine kinases. This triggers apoptosis (programmed cell death) and stops cancer cell proliferation.
- Multiple-Receptor Blockade: Erdafitinib inhibits FGFR1, FGFR2, FGFR3, and FGFR4 kinases at nanomolar concentrations (IC50 1.2 - 5.7 nM).
- FGFR23-Klotho Axis: FGFR1 inhibition interrupts FGF23 signaling, increasing phosphate reabsorption in the kidney tubules. This causes hyperphosphatemia, an indirect indicator of the medicine's biological activity.
- Selectivity: It is much more selective for the FGFR family than for other kinases such as RET, CSF1R, PDGFRA, PDGFRB, FLT3, and VGEFR2.

Balversa is the first successful example of personalized targeted treatment in bladder cancer. It offers a marked survival benefit in FGFR-mutant patients who do not respond to immunotherapy or have developed resistance to chemotherapy.
Balversa treatment requires precise biomarker guidance. Treatment starts with 8 mg, 1 time daily (two 4 mg tablets). Swallow the tablets whole with a glass of water, with or without food.
Day 14-21 Titration: Measure serum phosphate on day 14. If phosphate is < 9.0 mg/dL and there are no serious drug-related side effects, increase the daily dose to 9 mg (three 3 mg tablets).
- Serum Phosphate 5.5 - 6.9 mg/dL (Target Range): Indicates that the medicine is working; continue the current dose.
- Serum Phosphate 7.0 - 8.9 mg/dL: Start a phosphate-restricted diet and monitor until serum phosphate is < 5.5 mg/dL.
- Serum Phosphate 9.0 - 10.0 mg/dL: Add phosphate binders and monitor weekly until the level is < 5.5 mg/dL.
- Serum Phosphate > 10.0 mg/dL OR Tissue Calcinosis: Withhold Balversa. When serum phosphate falls to < 5.5 mg/dL, resume at the next lower dose level (8 mg or 6 mg).
- Kidney / Liver Impairment: No dose adjustment is needed for mild or moderate kidney impairment; clinical data are limited in severe kidney impairment (eGFR < 30 mL/min).
Absorption: After oral administration, peak plasma concentration is reached in an average of 2.5 hours (Tmax range 2-6 hours). Food does not affect absorption or AUC.
Distribution: The apparent volume of distribution at steady state is approximately 29 L. Plasma protein binding, primarily to alpha-1-acid glycoprotein, is 99.6%.
Metabolism: Erdafitinib is metabolized mainly in the liver by CYP2C9 (39%) and CYP3A4 (20%). Unmetabolized erdafitinib is the main circulating component in plasma.
Excretion: Of a single dose, 69% is excreted in feces (unchanged drug and metabolites) and 19% in urine. The mean elimination half-life (t1/2) is 56 hours.
- Strong CYP2C9 and CYP3A4 Inhibitors: Increase erdafitinib exposure and toxicity. Avoid concomitant use or select an alternative (for example, fluconazole or ketoconazole).
- Strong CYP2C9 and CYP3A4 Inducers: Lower Balversa plasma concentrations and reduce efficacy (for example, rifampin and St. John's wort). Concomitant use is not recommended.
- Medicines That Increase Serum Phosphate: Potassium phosphate supplements, vitamin D analogues, or laxatives multiply the risk of hyperphosphatemia. Stop these supplements when treatment begins.
- OCT2 Substrates: Erdafitinib may inhibit the renal OCT2 transporter. Monitor blood glucose and drug levels because it may affect the renal clearance of medicines such as metformin.
- Very Common (≥40%): Hyperphosphatemia (76%), stomatitis/mouth sores (56%), fatigue/asthenia (54%), diarrhea (47%), dry mouth (45%), nail disorders (43%), and altered taste/dysgeusia (42%).
- Laboratory Abnormalities: Increased creatinine, decreased sodium, decreased hemoglobin, increased ALT/AST, and decreased phosphate (rebound reactions).
- Grade 3-4 Side Effects: Stomatitis (14%), nail dystrophy (6%), palmar-plantar erythrodysesthesia/hand-foot syndrome (5%), and hyperphosphatemia (5%).
- Eye Toxicity (Central Serous Retinopathy - CSR): Approximately 25% of patients may develop central serous retinopathy/retinal pigment epithelial detachment. Blurred vision, visual field loss, and floaters may occur. Before treatment, monthly during the first 4 months, and then every 3 months, an eye examination including OCT (Optical Coherence Tomography) is mandatory. If CSR develops, interrupt treatment or reduce the dose.
- Hyperphosphatemia and Tissue Calcification: May cause calcium-phosphate deposition in soft tissues. Serum phosphate should be monitored routinely.
- Skin and Nail Toxicity: Separation of the nail from its bed (onycholysis), paronychia, and severe hand-foot syndrome may develop. Protective moisturizers are recommended.
- Embryo-Fetal Toxicity: Administration during pregnancy may cause fetal loss and malformations. Women of childbearing potential should use effective contraception during treatment and for 1 month after the last dose; male patients should use effective contraception for 1 month.
Acquired treatment resistance is associated with secondary gatekeeper mutations in the FGFR3 kinase domain, such as FGFR3 V555M/L or N540S. Compensatory activation of alternative receptor tyrosine kinase pathways (HER2, MET, or the PI3K/AKT/mTOR axis) and bypass signaling are also major factors in erdafitinib resistance.
Licensing and Import Status: Balversa (erdafitinib) can be imported into Turkey through the approved/prescribable medicines and Overseas Medicines List of the Turkish Ministry of Health's Turkish Medicines and Medical Devices Agency (TİTCK). Authorized medical oncology specialists prescribe it.
Biomarker Requirement: In Turkey, use of Balversa requires demonstration of an FGFR3 mutation or fusion in a tumor tissue sample using a validated NGS or PCR test.
Reimbursement Status: Balversa is **not included** in the List of Reimbursable Medicines under the Social Security Institution's (SGK) Health Implementation Communiqué (SUT). Patients pay for it themselves or seek payment from SGK through legal proceedings (interim injunction/lawsuit).
An important economic detail stands out in Balversa pricing:
- Starting Dose (8 mg/day): Take 2 tablets of 4 mg daily. A 28-day course requires 56 tablets of 4 mg.
- If 4 mg, 28-tablet packs are selected, buying 2 packs costs ₺799730.72 in total.
- However, 4 mg, 56-tablet pack selection offers the lower cost of ₺302713.05 when a single pack is chosen. This pack selection substantially reduces treatment costs.
- Increased Dose (9 mg/day - After Titration): When the dose increases to 9 mg/day on day 14 according to phosphate levels, take 3 tablets of 3 mg daily. The 3 mg, 56-tablet formulation, priced at ₺302713.05, is used for maintenance treatment.
- Have FGFR Testing: An FGFR3 genetic mutation test on bladder cancer tissue must be positive before the medicine can be used.
- Do Not Miss Eye Examinations: Before starting treatment, have a detailed retinal examination including Optical Coherence Tomography (OCT).
- Monitor Your Phosphate: Have blood phosphate measured on day 14 of treatment; your doctor may increase the dose to 9 mg.
- Review Pharmacy Supply and Pack Options: Pay attention to the prescribed dose and pack size (56-tablet packs rather than 28-tablet packs) when managing costs.
Urothelial carcinomas have a high mutation burden. FGFR alterations are associated with an immunologically cold tumor microenvironment, leading to lower responses to immune checkpoint inhibitors (anti-PD-1) in FGFR3-mutant patients. Balversa fills this treatment gap by directly inhibiting the kinase in this biological blind spot.
The phase 3 THOR study demonstrated clear overall survival (OS) and progression-free survival (PFS) gains. The response rate was 4 times higher than with chemotherapy (35.3% vs 8.5%). It offers the convenience of oral administration.
Frequent eye toxicity (CSR) requires repeated OCT monitoring. Managing hyperphosphatemia requires a strict diet and potentially phosphate binders. The high cost creates a barrier to access.
Interpret serum phosphate as a "pharmacodynamic biomarker" rather than simply a toxicity. Increasing the dose to 9 mg when serum phosphate is <9.0 mg/dL on day 14 improves clinical efficacy.
Ongoing clinical trials focus on erdafitinib's place in combination treatments with enfortumab vedotin (EV) or pembrolizumab and its potential for earlier-line use.
