FDA Approves Nivolumab–Ipilimumab Combination for Liver Cancer

In this article
On April 11, 2025, the Food and Drug Administration (FDA)approved nivolumab (Opdivo) and ipilimumab (Yervoy) in combination for adults with unresectable or metastatic hepatocellular carcinoma (HCC, liver cancer), as first-line systemic treatment .
Why Is This Approval Important?
Liver cancer, particularly hepatocellular carcinoma (HCC) , is among the leading causes of cancer-related death worldwide. Most patients are diagnosed at an advanced stage, often without an opportunity for surgery. Survival also remains limited with standard systemic treatments such as chemotherapy.
Consequently, immunotherapiesthat activate the immune system to fight tumors have emerged as a promising area in liver cancer treatment.
With a 36% response rate and median overall survival of 23.7 months, nivolumab + ipilimumab received FDA approval for first-line advanced liver cancer treatment as, according to the indirect comparisons shown at the end of this article, what appears to be the most effective first-line treatment to date .
CHECKMATE-9DW: The Phase 3 Trial Behind the New Approval
This development is based on CHECKMATE-9DW , a multicenter, randomized phase 3 clinical trial. Its design included:
- A total of 668 patients .
- All patients had:
- Histologically confirmed HCC ,
- Child-Pugh A liver function, ECOG PS 0-1 performance status,
- And no previous systemic treatment.
Treatment arms:
- Experimental arm:
- Nivolumab (1 mg/kg) + ipilimumab (3 mg/kg) every 3 weeks for a total of 4 doses
- Followed by nivolumab 480 mg IV every 4 weeks as a single agent
- Control arm:
- Lenvatinib or sorafenib, at the investigator's discretion
Overall Survival: A Notable Improvement
The study's primary endpoint: Overall survival (OS)
| Median OS | Confidence Interval (CI) | |
|---|---|---|
| Nivolumab + Ipilimumab | 23.7 months | 95% CI: 18.8 – 29.4 months |
| Lenvatinib/Sorafenib | 20.6 months | 95% CI: 17.5 – 22.5 months |
Hazard ratio (HR): 0.79 | p < 0.018
The difference demonstrates the experimental arm's effectiveness and also better-than-expected performance in the control arm. This may reflect the effects of patient selection, supportive care, and follow-up quality in contemporary clinical trials.
Response Rate: An Almost 3-Fold Increase
Secondary endpoint: Objective response rate (ORR)
| ORR (%) | Confidence Interval (CI) | |
|---|---|---|
| Nivolumab + Ipilimumab | 36.1% | 95% CI: 31.0% – 41.5% |
| Lenvatinib/Sorafenib | 13.2% | 95% CI: 9.8% – 17.3% |
p < 0.0001 indicated high statistical significance.
This finding shows that the immunotherapy combination extended survival while also producing tumor shrinkage in a higher proportion of patients .
Adverse Reactions and Safety Profile
As with most immunotherapies, activating the immune system can produce adverse reactions. The most frequently reported reactions (>20%) were:
- Rash
- Itching (pruritus)
- Fatigue
- Diarrhea
The vast majority of adverse reactions can be managed effectively at experienced centers.
Dosing Schedule: How Is It Given?
🔹 Induction Phase (First 12 Weeks):
- Nivolumab: 1 mg/kg
- Ipilimumab: 3 mg/kg
- IV infusion every 3 weeks, for a total of 4 doses
🔹 Maintenance Phase:
- Nivolumab:
- 240 mg IV → every 2 weeks or
- 480 mg IV → every 4 weeks
Clinical Implications and Future Perspectives
The combination of nivolumab + ipilimumab has shown that immunotherapy is also effective as first-line treatment in HCC with compelling evidence.
Previously granted accelerated approval in the second-line setting, this regimen has now, through CHECKMATE-9DW, become a standard treatment alternative .
This development expands treatment options and also represents a promising new treatment paradigm for liver cancer .
