What Is Ozempic (Semaglutide)? — Introduction and Why It Matters
Ozempic®, containing semaglutide, is a long-acting GLP-1 receptor agonist (GLP-1RA) administered under the skin 1 time weekly. Although sometimes called a “weight-loss injection,” Ozempic's primary approved indication is glycemic control in Type 2 diabetes; weight loss is a secondary consequence of this drug class's physiological effects. A separately approved, higher-dose (2.4 mg) semaglutide formulation for obesity is marketed as Wegovy®—the difference in brand and dose matters.
Ozempic® (semaglutide) was developed and is manufactured by the Denmark-based pharmaceutical company Novo Nordisk.
Semaglutide increases meal-stimulated insulin secretion in a glucose-dependent manner, suppresses glucagon, slows gastric emptying, and reduces appetite. This can produce a significant HbA1c reduction and substantial weight loss depending on dose and duration. It is also indicated to reduce the risk of major adverse cardiovascular events (MACE) in people with Type 2 diabetes and established cardiovascular disease and—from 2025—to slow progression of chronic kidney disease (see the recent developments box below).
Highlights
- Class: GLP-1RA · Administration: 1 s.c. dose weekly · Brands: Ozempic® (T2D, injection) / Wegovy® (Obesity) / Oral Ozempic® tablets also available in the US from 2026
- Strong glycemic control: HbA1c reduction of ~1.5-2.0% (depending on dose/background treatment)
- Weight management: Clinically meaningful weight loss through reduced appetite and increased satiety
- Cardiovascular and kidney benefits: Reduced MACE risk in T2D + established cardiovascular disease; from 2025, an indication to slow kidney function loss in T2D + chronic kidney disease
- Ease of use: A single weekly dose, practical titration
- Not covered by SGK in Türkiye: Purchased out of pocket even for the Type 2 diabetes indication
Who May Be Considered? & Expectations
- In T2D: As monotherapy or in combination when glycemic control is inadequate.
- In T2D with high cardiovascular risk: To reduce the risk of major adverse cardiovascular events.
- In T2D + chronic kidney disease: To slow kidney function loss (approved from 2025; see below).
- Expected effects: Satiety with smaller portions, reduced appetite, gradual weight loss.
Note: Wegovy®, rather than Ozempic®, is used for obesity treatment. Treatment should be planned under medical supervision, considering individual risks and concomitant medications.
Recent developments (2020-2026)
- 16 January 2020 — Cardiovascular indication: Based on SUSTAIN-6 data, the FDA approved Ozempic to reduce major adverse cardiovascular event risk in adults with Type 2 diabetes and known cardiovascular disease (Rybelsus labeling was updated in the same action).
- 28 January 2025 — Chronic kidney disease (FLOW trial): The FDA approved Ozempic to reduce the risk of sustained eGFR decline, kidney failure, and cardiovascular death in adults with Type 2 diabetes and chronic kidney disease—a major development making Ozempic the first GLP-1RA with a dedicated indication in this setting.
- February 2025 — Removal from the shortage list: The FDA removed semaglutide from its official drug shortage list, largely ending the production of “essentially identical” compounded semaglutide.
- 5 December 2025 — Counterfeit product warning: The FDA issued a consumer warning about counterfeit Ozempic 1 mg pens identified in the legitimate US drug supply chain. In Türkiye too, it is important not to obtain medicine without a prescription/from unreliable sources (the internet, purchases abroad, etc.).
- February-May 2026 — Oral Ozempic tablets: In February 2026, the FDA approved a new oral tablet formulation (1.5/4/9 mg) under the Ozempic brand; it launched in the US in May 2026. This is a rebranded/expanded version of the oral semaglutide line previously sold as Rybelsus; the injectable and tablet formulations now share the “Ozempic” brand umbrella but are different products. This tablet formulation is currently unavailable in Türkiye.
Mechanism of Action
1) Pancreatic effects
GLP-1R activation increases glucose-dependent insulin secretion and suppresses glucagon, lowering postprandial blood glucose.
2) Gastrointestinal and central effects
Gastric emptying slows (particularly early on), appetite decreases (hypothalamic satiety signals), and energy intake falls.
3) Pharmacokinetic design
Fatty acid modification and albumin binding yield t½ ≈ 1 week, enabling weekly dosing and low glycemic variability.
FDA Approval Process — Timeline
- 5 December 2017: Ozempic® — Initial FDA approval for glycemic control in Type 2 diabetes.
- 16 January 2020: Cardiovascular risk reduction indication based on SUSTAIN-6 data.
- 28 March 2022: FDA approval of the 2 mg/week dose.
- 28 January 2025: Kidney-protective indication in Type 2 diabetes + chronic kidney disease (FLOW trial).
- February 2026: Approval of oral Ozempic tablets (1.5/4/9 mg)—launched in the US in May 2026.
What Are Ozempic's Price, Authorization, and Reimbursement Status?
Authorization / Product
Ozempic's TİTCK authorization in Türkiye is reported as dated 17.05.2022 (although reasonably consistent with secondary sources, this date could not be directly confirmed from TİTCK's primary record; consult TİTCK's list of authorized products for the exact date).
- Dose formulations sold in Türkiye: 0.25 mg/0.5 mg dual-dose pen; 1 mg pen.
- Not sold in Türkiye: The 2 mg/week pen, available in the US since 2022, is not marketed in Türkiye. Oral Ozempic tablets, launched in the US in 2026, are also currently unavailable in Türkiye.
Price
| Dose | Pack | Price (Including VAT) |
|---|---|---|
| Ozempic 0.25 mg pen | 1 unit | 10,643.48 TL |
| Ozempic 0.5 mg pen | 1 unit | 10,643.48 TL |
| Ozempic 1 mg pen | 1 unit | 10,643.48 TL |
Note: The nearly identical prices of all three doses are not an error—Türkiye's reference pricing system generally prices different-dose pens within the same drug family at the same reference price. No SGK reimbursement is available —even for the Type 2 diabetes indication, it is purchased out of pocket. This information does not replace a doctor/pharmacist and is for general information; ask your pharmacist for the current price.
Authorization in Türkiye
Type 2 Diabetes Mellitus:
- As an adjunct to diet and exercise in adults with inadequately controlled Type 2 DM,
- As monotherapy when metformin is not tolerated or is contraindicated,
- In addition to other medicinal products for diabetes treatment.
Cardiovascular Risk Reduction: Indicated to reduce the risk of major adverse cardiovascular events (MACE) in adults with Type 2 DM and known cardiovascular disease.
Note: At the time this article was prepared, it could not be confirmed whether the chronic kidney disease indication approved by the FDA in January 2025 had been incorporated into Türkiye's official product label (SmPC). Consult your pharmacist or physician about the current scope of indications.
Dose Range and How to Use It
| Stage | Weekly Dose (s.c.) | Duration (Minimum) | Note |
|---|---|---|---|
| Starting | 0.25 mg | 4 weeks | Tolerability; limited glycemic effect, preparation for titration |
| Gradual Increase | 0.5 mg | ≥4 weeks | Monitoring based on goals |
| Higher Dose | 1 mg (2 mg if needed—US) | ≥4 weeks/step | According to glycemic/weight goals and tolerability; the 2 mg dose is not sold in Türkiye |
- Injection sites: Abdomen, thigh, upper arm (rotate sites).
- Keep the same weekly day: If changing it, leave at least 48-72 hours between doses.
- Missed dose: May be administered within 120 hours (5 days); otherwise skip it and return to the scheduled day.
- With insulin/sulfonylureas: The concomitant drug's dose may be reduced because of hypoglycemia risk.
Absorption, Distribution, Metabolism, and Excretion (ADME)
| Parameter | Information |
|---|---|
| Absorption | After s.c. administration, Tmax ≈ 1-3 days; high absolute bioavailability |
| Distribution | Very high albumin binding; low apparent Vd → prolonged action |
| Metabolism | Proteolytic breakdown and β-oxidation; no clinically significant CYP metabolism |
| Excretion | In urine and feces as small peptides/metabolites |
| Half-life | ~7 days—supports weekly dosing; steady state reached in ~4-5 weeks |
| Special populations | No significant difference expected in mild-to-moderate kidney/liver impairment; clinical monitoring in advanced impairment |
Drug Interactions
- Delayed gastric emptying: May affect absorption of some oral drugs; clinical monitoring is recommended for drugs with a narrow therapeutic window.
- Oral contraceptives: No clinically significant interaction is expected; nevertheless, use caution during early titration.
- Warfarin/anticoagulants: No known direct interaction has been reported; precautionary INR monitoring may be considered because of altered gastric emptying.
- Insulin and sulfonylureas: Hypoglycemia risk increases; dose reduction may be needed.
- Do not use concurrently with other semaglutide products: Do not use alongside Wegovy®, Rybelsus®, or oral Ozempic® tablets sold in the US from 2026—all contain the same active ingredient; the doses should not be combined, as this leads to duplicate/incorrect dosing.
Side-Effect Profile
Common: Nausea, vomiting, diarrhea/constipation, abdominal pain, reduced appetite; headache, fatigue; injection-site reactions.
Requiring Attention: Gallbladder disorders (cholelithiasis/cholecystitis); acute kidney injury (particularly with gastrointestinal losses/dehydration); worsening diabetic retinopathy (related to rapid glycemic improvement).
Serious / Rare: Pancreatitis (stop the drug and seek assessment for severe, persistent abdominal pain); hypersensitivity reactions; a class warning for thyroid C-cell tumors (animal data; limited long-term human data).
Thyroid C-cell tumor warning — boxed warning
This warning is based on thyroid C-cell tumors observed in rodents (rats); long-term human data are limited. In people with a personal or family history of medullary thyroid carcinoma (MTC) or MEN2 syndrome, the drug is contraindicated. This is a standard class warning applying to all GLP-1RAs.
Beware of counterfeit products
In December 2025, the FDA issued a consumer warning about counterfeit Ozempic 1 mg pens identified in the US supply chain. Although the warning directly concerns the US market, the general principle applies everywhere: obtain Ozempic only from a pharmacy, with a prescription and in its original packaging; never buy it online or from unreliable sources.
Tolerability: Gradual titration and administration after meals may help reduce gastrointestinal side effects.
Situations Requiring Caution
- Contraindicated: Personal/family history of medullary thyroid carcinoma (MTC) or MEN-2; severe hypersensitivity.
- History of pancreatitis: Close monitoring; immediate assessment if symptoms develop.
- Eyes: If diabetic retinopathy is present, arrange ophthalmologic review at baseline and during rapid HbA1c reduction.
- Kidneys: Risk of acute kidney injury with gastrointestinal losses/dehydration; pay attention to fluid management.
- Pregnancy/planned pregnancy: Not recommended; because of the long half-life, discontinuation at least 2 months before planned pregnancy is advised.
- Concomitant treatments: Review insulin/sulfonylurea doses; establish a routine of administration on the same day at the same time.
Current oncology research
Ozempic is not a cancer drug and its label carries a class warning for thyroid C-cell tumors—however, current human data do not provide evidence that GLP-1 agonists increase thyroid cancer risk. See our article addressing this common misconception: No! Weight-Loss Injections Do Not Cause Thyroid Cancer . In a separate research area, obesity and Type 2 diabetes themselves are known risk factors for various cancers; some observational studies have reported early findings suggesting that GLP-1 drugs reduce metastasis risk in certain cancers. However, these are observational or preclinical data that do not yet establish causation.
