Keytruda (Pembrolizumab) and Lenvima (Lenvatinib) Treatment Regimen

In this article
What Is Pembrolizumab + Lenvatinib?
Pembrolizumab + lenvatinib combines PD-1 blockade with anti-VEGFR/FGFR TKI activity. Its main uses are:
1) Advanced endometrial carcinoma (pMMR or non-MSI-H) after prior systemic treatment in adults who are not candidates for curative surgery/RT;
2) Advanced/metastatic RCC in the first-line setting. Efficacy was demonstrated in the Phase III trials KEYNOTE-775 (EC) and CLEAR/KEYNOTE-581 (RCC).
⚙️ How Does It Work?
- Pembrolizumab: Blocks PD-1/PD-L1 interaction, increasing T-cell activation → stronger antitumor immunity.
- Lenvatinib: Inhibits VEGFR-1/2/3, FGFR-1/2/3/4, PDGFR-α, RET and KIT; reduces angiogenesis and the immunosuppressive microenvironment.
- Synergy: Vessel normalization + increased T-cell infiltration ↑ → enhanced PD-1 blockade.
🎯 Treatment Goals
🧬 Tumor Control
Immune checkpoint blockade + antiangiogenic suppression increase objective response and disease control rates.
⏳ Delaying Progression (PFS)
RCC (CLEAR): Median PFS: 23.9 months vs sunitinib 9.2 months (HR 0.39).
📊 Extending Overall Survival (OS)
- EC (KEYNOTE-775): Median OS in the pMMR population: 17.4 vs 12.0 months (HR 0.68); ITT: 18.3 vs 11.4 months (HR 0.62).
- RCC (CLEAR): OS superior to sunitinib (initial analysis HR 0.66); the benefit persisted at long-term follow-up.
🌿 Quality of Life
Lower symptom burden and reduced steroid requirements may help maintain daily activities .
🧾 Treatment Indications & Guideline Position
✅ Approved Indications
- Endometrial Carcinoma (EC): pMMR or non-MSI-H; after prior systemic treatment in adults ineligible for curative surgery/RT.
- RCC (1st Line): Across all IMDC risk groups, pembrolizumab + lenvatinib is among the preferred options.
📚 Guideline Note
In endometrial carcinoma, first-linetreatment with pembrolizumab + carboplatin/paclitaxel is separately approved; pembro+lenva is used mainly after platinum treatment.
💉 Dose and Administration Schedule
| Component | Standard Dose | Interval | Administration | Duration |
|---|---|---|---|---|
| Pembrolizumab | 200 mg or 400 mg | Every 3 weeks or every 6 weeks | IV infusion ~30 minutes | ≤ 24 months (progression/tolerance) |
| Lenvatinib | 20 mg/day | Daily | Oral (single dose; with or without food) | Until progression/unacceptable toxicity; after 2 years of combination treatment in RCC, may continue alone as monotherapy |
🔄 Administration Sequence
On the first day, give pembrolizumab infusion (≈30 minutes); the same day or next morning, start daily lenvatinib.
-
Take a single daily dose ; with or without food , at the same timeeach day. Swallow capsules whole (do not crush/chew). Patients unable to swallow may receive a suspension prepared in water or apple juice (through a feeding tube if needed).
-
If a dose is missed and the next dose is due in ≤12 hours , skip the missed doseand take the next dose at its usual time.
📌 Pretreatment Checks
- Blood pressure (including home measurements), TSH/fT4, AST/ALT/ALP/bilirubin, creatinine/eGFR, complete blood count
- Urine protein (dipstick ± 24 hours)
- HBV/HCV assessment; history of severe autoimmune disease/transplantation review
- If needed, ECG + electrolytes (QT risk; concomitant QT-prolonging medications)
- Pregnancy test and counseling on effective contraception .
Note: In severe kidney/liver impairment, the recommended starting dose of lenvatinib is 10 mg/day .
⚠️ Special Situations Requiring Attention
🩺 Hypertension & Proteinuria (Lenvatinib)
- Bring hypertension under control before starting ; during the first 2 months, monitor closely .
- G3 HT (despite optimal treatment): withhold/reduce dose; G4: permanently discontinue.
- Proteinuria ≥2 g/24 hours: withhold/reduce dose; nephrotic syndrome: permanently discontinue.
🧠 Immune-Related AEs (Pembrolizumab)
- Pneumonitis, colitis, hepatitis, nephritis and endocrinopathies (thyroiditis/hypophysitis/adrenal disorders) may occur.
- G2–3: withhold + systemic steroids; taper gradually after improvement. G4: permanently discontinue in most cases.
🩻 Surgery/Dental Procedures
- Withhold lenvatinib before planned major surgery, ≥1 week beforehand ;after major surgery, wait ≥2 weeks and until wound healing is complete before restarting.
👶 Pregnancy/Breastfeeding & QT
- Lenvatinib may cause embryo-fetal toxicity ; effective contraception is essential.
- QT prolongation risk: avoid concomitant QT-prolonging drugs; correct electrolytes and monitor ECG if needed.
🤒 Selected Side Effects
RCC – CLEAR: Any-grade AEs ≈ 100%; G≥3 AEs ≈ 82%. EC – KEYNOTE-775: G≥3 AEs ≈ 89%. Common events include hypertension, diarrhea, appetite/weight loss, proteinuria, hypothyroidism, mucositis and rash.
| Side Effect | Frequency (Summary) | Management (Label) |
|---|---|---|
| Hypertension (lenva) | Very common | G3: withhold/reduce dose; G4: discontinue |
| Proteinuria (lenva) | Common | ≥2 g/24 hours: withhold/reduce; nephrotic syndrome: discontinue |
| irAEs (pembro) | Common (highly variable) | G2–3: interrupt + steroids; G4: discontinue in most cases |
| Diarrhea/loss of appetite, HFSR, mucositis | Common | Symptomatic support + lenva dose reduction steps (14→10→8 mg) |
ℹ️ Important Note
Side effects vary between patients. When severe toxicity is suspected, early reporting and dose modification are critical for treatment continuity.
🔎 Follow-Up & Monitoring
| Parameter | Frequency | Threshold/Note |
|---|---|---|
| Blood pressure | After week 1, for 2 months, every 2 weeks, then at least monthly6 | G3: withhold/reduce; G4: discontinue |
| TSH/fT4 | Monthly | Hypothyroidism is common; replacement may be needed |
| Urine protein | Every 4–6 weeks | ≥2 g/24 hours: withhold/reduce |
| LFT/renal/complete blood count | Every 3–6 weeks | Withhold/reduce according to hepatic/renal toxicity |
| Imaging | Every 8–12 weeks | Earlier assessment if clinical deterioration occurs |
🧮 Dose Modification: Practical Thresholds
- Lenvatinib Reduction Steps: 20 → 14 → 10 → 8 mg/day (RCC/EC).
- HT (G3): withhold/reduce; G4: permanently discontinue. Proteinuria ≥2 g/24 hours: withhold/reduce; nephrotic syndrome: discontinue.
- Pembrolizumab: do not reduce the dose; G2–3 irAE → withhold + steroids; G4 → usually discontinue.
- Severe Kidney/Liver Impairment: Starting lenvatinib dose: 10 mg/day.
📌 Clinical Nuances
- Endometrial Carcinoma: Confirming pMMR is important; for MSI-H/dMMR disease, single-agent ICI options may also be considered.
- RCC: After completing 2 years of combination treatment, lenvatinib monotherapy may continue.
🔄 Drug Interactions
Pembrolizumab
- Conventional CYP-mediated interactions are not expected (monoclonal antibody).
- High-dose immunosuppressants (such as steroids) may reduce efficacy; for irAE management they are indicated.
Lenvatinib
- QT-prolonging drugs increase arrhythmia risk ↑; avoid if possible or monitor ECG + electrolytes.
- Anticoagulants/antiplatelets increase bleeding risk ↑ → monitor carefully.
- For planned surgery/dental procedures, follow the timing rulesabove.
