Enhertu (Trastuzumab Deruxtecan) and Pertuzumab (Perjeta) Treatment Regimen

In this article
A New Standard: Enhertu + Pertuzumab
Enhertu (Trastuzumab Deruxtecan) + Pertuzumabis a next-generation regimen for first-line treatment of metastatic or unresectable HER2-positive breast cancer , replacing conventional chemotherapy combinations (taxane + trastuzumab + pertuzumab), with FDA approval in December 2025.
The combination aims to reduce conventional chemotherapy's systemic side effects while using tumor-specific “smart bomb” (ADC) technology and “dual HER2 blockade” to take progression-free survival (PFS) to unprecedented levels.
- Indication: Metastatic HER2(+) Breast Cancer – 1st Line.
- Mechanism: Antibody–Drug Conjugate (ADC) + Dimerization Inhibitor.
- Evidence: DESTINY-Breast09 Trial (PFS Advantage).
- Interstitial Lung Disease (ILD): The most critical risk.
- Cardiotoxicity: Reduced LVEF.
- Gastrointestinal: Nausea, vomiting (high emetic risk).
- Hematologic: Neutropenia, anemia.
- Extend PFS (time without progression) beyond 40 months.
- Avoid conventional taxane chemotherapy toxicity.
- Potential intracranial activity (brain metastases).
Key Indications and Patient Selection
HER2-Positive Metastatic Breast Cancer (1st Line)
For patients with tumor tissue confirmed as IHC 3+ or ISH (+) HER2-positive, with no prior systemic treatment in the metastatic setting (or >6 months after adjuvant treatment), this is a new standard of care option.
Comparison With Conventional THP
Suitable and Higher-Risk Patient Profiles
- ECOG performance status 0-1.
- Healthy lung parenchyma (no COPD or fibrosis history).
- Normal cardiac function (LVEF above 50%).
- History of ILD: Prior drug-induced or radiation pneumonitis (Enhertu may be contraindicated).
- Cardiac Risk: History of heart failure.
- Renal/Hepatic Impairment: May require dose adjustment or close monitoring.
Enhertu (Trastuzumab Deruxtecan - T-DXd)
Enhertu is an Antibody–Drug Conjugate (ADC). A potent chemotherapy agent (topoisomerase I inhibitor) is attached to a HER2-binding antibody, trastuzumab.
- Trojan Horse Effect: The drug enters the cell and releases its payload inside.
- Bystander Effect: The drug can cross the cell membrane and kill neighboring HER2-negative cancer cells, critical in heterogeneous tumors.
Pertuzumab (Perjeta)
A monoclonal antibody preventing HER2 dimerization (pairing). It blocks HER2 from combining with HER3 to send powerful growth signals. Together with Enhertu, it provides complete two-pronged receptor blockade .
The phase 3 trial underpinning approval was DESTINY-Breast09. In this global study of more than 1100 patients:
- PFS (Progression-Free Survival): Median PFS with Enhertu + Pertuzumab was 40.7 months , versus 26.9 months with standard THP.
- Objective Response Rate (ORR): Tumor shrinkage rates were higher in the Enhertu arm.
- Intracranial Effect: Systemic control was also demonstrated in patients with stable brain metastases.
Treatment is generally given every 3 weeks (21-day cycles) .
Regimen Details
- 1. Enhertu (T-DXd):
- Dose: 5.4 mg/kg IV infusion.
- Duration: First dose over 90 minutes; subsequent doses over 30 minutes if well tolerated.
- Diluent: 5% Dextrose Only (Do not use normal saline).
- 2. Pertuzumab:
- Loading Dose (Cycle 1): 840 mg IV (60 minutes).
- Maintenance Dose (Cycle 2 onward): 420 mg IV (30-60 minutes).
- May be given after Enhertu.
- Enhertu has high/moderate emetic risk. Give a two- or three-drug antiemetic regimen (5-HT3 antagonist + Dexamethasone ± NK1 inhibitor).
- Premedication may be needed to prevent infusion reactions.

A. Interstitial Lung Disease (ILD) / Pneumonitis
- Symptoms: Cough, shortness of breath, fever, new radiological opacities.
- Management: Stop the drug as soon as suspected. For symptomatic cases (Grade 2+), permanently discontinue and start high-dose steroids.
B. Cardiotoxicity
Both drugs, particularly in combination, may affect cardiac muscle function (LVEF). During treatment, echocardiography/MUGA every 3 months is mandatory.
C. Other Common Side Effects
- Nausea and Vomiting: Effective antiemetic treatment is essential.
- Alopecia (Hair Loss): Although taxane-free, Enhertu may cause hair loss in 40-50%.
- Neutropenia: Reduced immune cells, increasing infection risk.
- Diarrhea: Usually mild to moderate (Pertuzumab-related).
Pertuzumab doses are not reduced: it is either given or stopped. Enhertu may be reduced according to toxicity.
| Toxicity / Situation | Management Strategy |
|---|---|
| ILD (Asymptomatic – Grade 1) | Withhold. Consider steroids. Restart after recovery. |
| ILD (Symptomatic – Grade 2+) | Enhertu PERMANENTLY DISCONTINUE. Start systemic steroids. |
| Neutropenia (Grade 3-4) | Delay treatment. After recovery, resume one dose level lower (5.4 -> 4.4 mg/kg). |
| LVEF Decline (40-45%) | Delay treatment. Reassess after 3 weeks. Permanently discontinue if not improved. |
This treatment combines two targeted drugs binding to the “antennas” (HER2) on breast cancer cells. Pertuzumab stops the antennas from signaling, while Enhertu binds and releases a potent drug inside the cell.
Treatment is given by IV infusion every 3 weeks . It may not cause complete hair loss like conventional chemotherapy, although thinning can occur, and generally causes less fatigue. However, if you develop a dry cough or shortness of breath , notify your doctor immediately, as this may indicate lung sensitivity.
- Pertuzumab: Authorized in Türkiye. SGK covers its conventional THP combination, but the new Enhertu combination is not yet reimbursed.
- Enhertu: Authorized by TİTCK. For 1st-line use, off-label approval or additional applications may generally be needed. Not reimbursed by SGK.
- For the current situation, patients may pursue legal action after the off-label application process.
