What Is Datopotamab Deruxtecan (Datroway)? FDA Approvals, Authorization, Price and Reimbursement Status in Turkey

What Is Datopotamab Deruxtecan (Datroway)? FDA Approvals, Authorization, Price and Reimbursement Status in Turkey
Updated: July 2026. This monograph was prepared using official FDA approval notices, European Medicines Agency (EMA) product information, manufacturer statements and peer-reviewed literature, and updated with the latest FDA approval (May 22, 2026). It serves as a reference and is revised as approval and authorization statuses change.

Datroway (datopotamab deruxtecan)is used to treat breast and lung cancer and is a TROP2-targeted antibody-drug conjugate (ADC) — a targeted delivery system carrying a chemotherapy payload directly to tumor cells. Its story has expanded rapidly over the past year and a half: initial approval came in early 2025, followed by a lung cancer indication that year, and first-line approval in triple-negative breast cancer in May 2026 substantially broadened its scope.

However, the two most important messages of this monograph are: (1) Datroway still has no Ministry of Health authorization in Turkey — it is not available in the country, is not reimbursed by SGK and has no established Turkish price. (2) The drug's first breast cancer approval set a controversial regulatory precedent in oncology: the supporting trial delayed progression but did not prolong survival. Interestingly, the trial supporting its most recent approval did achieve that. This distinction is examined in detail below.

📋 Drug Profile — Datroway (Datopotamab Deruxtecan)

Information Corrected and Added in This Update
  • The Initial Approval Date Was Corrected. Some sources give the initial FDA approval as “April 19, 2024”; this is incorrect. Datroway's initial FDA approval was on January 17, 2025 . Developments in 2024 were not approvals, but acceptance of biologics license applications (BLAs) and announcements of trial results.
  • A New Indication Was Added (May 22, 2026): In triple-negative breast cancer, first-line approval. This most substantially expanded the drug's scope and was absent from earlier monographs.
  • A Critical Efficacy Nuance Was Added: The study supporting the first breast cancer approval did not demonstrate an overall survival benefit. This is discussed in detail later in the article.
  • Price Information Was Added (US list price and European status) — in the section below.
WHAT IS DATROWAY? THE TARGETED-DELIVERY PRINCIPLE

The main limitation of conventional chemotherapy is its lack of selectivity: it affects rapidly dividing healthy cells as well as cancer cells. Antibody-drug conjugates (ADCs) were designed to address this. The principle is simple: attach a potent cell-killing molecule to an antibody that recognizes a specific address label on the tumor cell surface. The antibody finds the address, carries the payload inside and releases the drug there.

In Datroway, that address label is TROP2 (trophoblast cell-surface antigen 2). TROP2 is overexpressed on many epithelial tumors, including breast and lung cancers.

How Does Datroway Work? Step by Step
  1. Targeting: The antibody component (humanized anti-TROP2 IgG1) binds TROP2 on the tumor cell surface.
  2. Internalization: The cell takes up the complex through endocytosis and transports it to digestive compartments called lysosomes.
  3. Payload Release: Enzymes in the lysosome cleave the cleavable tetrapeptide linker connecting the antibody to the cytotoxic payload, releasing DXd . (DXd is an exatecan derivative, with approximately 4 DXd molecules attached to each antibody.)
  4. DNA Damage: DXd inhibits topoisomerase I . This enzyme allows the DNA helix to unwind during replication. When blocked, DNA breaks accumulate and the cell undergoes programmed self-destruction (apoptosis).
  5. Bystander Effect: Released DXd can leak through the cell membrane and may therefore affect cells with low TROP2 expression, including neighboring tumor cells . This offers an advantage against tumor heterogeneity.
Antibody-drug conjugate: the targeted delivery chain 1. Targeting Antibody–TROP2 binding 2. Internalization The complex enters the cell (endocytosis) 3. Payload Released The linker is cleaved, DXd is released 4. DNA Damage Topoisomerase I is blocked 5. Apoptosis The cell destroys itself + Bystander Effect Free DXd can leak out of the cell and affect neighboring tumor cells with low TROP2 expression
Datroway's five-step mechanism and bystander effect, which offers an advantage against tumor heterogeneity. Schematic.
FDA APPROVAL HISTORY AND CURRENT INDICATIONS

Datroway currently has three separate FDA indications : two in breast cancer and one in lung cancer:

Date Indication Treatment Line / Conditions Supporting Evidence
January 17, 2025
(initial approval)
Unresectable or metastatic HR-positive, HER2-negative (IHC 0, 1+ or 2+/ISH−) breast cancer Not first line. Adults with advanced/metastatic disease previously treated with both endocrine therapy and chemotherapy in that setting TROPION-Breast01 (NCT05104866)
June 23, 2025 Locally advanced or metastatic EGFR-mutated non-small cell lung cancer (NSCLC) Not first line. Adults previously treated with EGFR-targeted therapy and platinum-based chemotherapy. Accelerated approval (based on response rate and duration; dependent on a confirmatory trial) Response rate / response duration data
May 22, 2026
(most recent)
Unresectable or metastatic triple-negative breast cancer (TNBC) First line — but only for people not eligible for a PD-1/PD-L1 inhibitor (immunotherapy) in this setting TROPION-Breast02 (NCT05374512), n=644

The most recent approval was granted through Project Orbis , with simultaneous review by international regulatory partners. With this indication, Datroway entered the NCCN guidelines for metastatic TNBC ineligible for immunotherapy as a Category 1 “preferred” first-line option .

AN HONEST ASSESSMENT: THE SURVIVAL QUESTION

This is the most important section of the monograph and is often absent from promotional drug information. The trials supporting Datroway's two breast cancer approvals differ substantially in efficacy.

HR 1.01TROPION-Breast01: no overall survival difference (18.6 vs 18.3 months)
6.9 / 4.9 monthsPFS benefit in the same trial — HR 0.63
23.7 / 18.7 monthsTROPION-Breast02: overall survival benefit demonstrated
TROPION-Breast01 (Supporting the HR-Positive/HER2-Negative Approval)
  • Showed a progression-free survival (PFS) benefit: median 6.9 versus 4.9 months; hazard ratio 0.63.
  • But did not show an overall survival (OS) benefit, a co-primary endpoint: median 18.6 versus 18.3 months; hazard ratio 1.01. In terms of survival, the drug was not superior to chemotherapy.
  • Nevertheless, the drug received full approval . Peer-reviewed literature described this as full approval granted despite failure of the co-primary overall survival endpoint, calling it the “first breast cancer drugto receive such approval,” and regarding it as a controversial precedent for both the FDA and EMA.
TROPION-Breast02 (Supporting the Triple-Negative Breast Cancer Approval) — A Different Picture
  • The study enrolled previously untreated patients in the metastatic setting who were ineligible for immunotherapy: 644 patients with TNBC were included, with investigator's choice of chemotherapy as the comparator.
  • Progression-free survival: median 10.8 versus 5.6 months.
  • Overall survival: median 23.7 versus 18.7 months — this trial therefore demonstrated a survival benefit.

This contrast has practical meaning for patients and physicians: The strength of Datroway's evidence differs between indications. First-line use in triple-negative breast cancer rests on robust survival data, whereas evidence in HR-positive/HER2-negative disease is primarily based on delaying disease progression . This does not make that indication worthless — delaying progression may matter for symptom control and quality of life — but it is not equivalent to a promise of longer life , and this distinction should inform treatment sequencing.

STATUS IN TURKEY AND PRICE
🇹🇷 Status in Turkey: The Drug Is Not Available

Datroway has no authorization from Turkey's Ministry of Health (TİTCK). This has three direct consequences: (1) The drug is not routinely available through pharmacies or hospitals in Turkey. (2) There is no SGK reimbursement. (3) There is also no established official selling price for Turkey — pricing follows the authorization process.

For Comparison: The other antibody-drug conjugate targeting TROP2, sacituzumab govitecan (Trodelvy), has been authorized in Turkey since January 2023. Thus, TROP2-targeted treatment is not entirely absent in Turkey, but Datroway itself is not accessible.

The table below shows the drug's costs in countries where it is available . These figures are list prices; what patients or insurers actually pay may differ because of discounts and reimbursement agreements.

Country / Region Authorization Status List Price per Vial Approximate Cost per 1 Cycle (3 Weeks)
US Authorized (since January 17, 2025) ~4891 US dollars / 100 mg single-use vial (manufacturer's official wholesale price statement, January 2025) ~20000–24500 US dollars
Weight-based dosing generally requires 4–5 vials per cycle for an average adult
European Union Authorized (granted in 2025) No publicly available, verifiable unit price could be found Cannot be reliably calculated (explanation below)
Turkey NONE Not established Not established
Why Can No Definite European Price Be Given?

Rather than simply a lack of data, this reflects the European pricing structure . For hospital-administered oncology medicines, many European countries enter into confidential discount agreements with manufacturers. The published list price, if any, therefore does not reflect the payer's actual cost and varies substantially between countries. In addition, for Datroway's current European unit price, no publicly accessible, verifiable source could be found at the time of this update.

For transparency, we do not provide an estimated figure. For precise information, consult the official medicine price list or payer in the patient's country. As an indication of scale only, ADCs in this class also cost tens of thousands of euros per cycle in Europe.

SIDE EFFECTS AND SAFETY

Although Datroway is targeted, its payload is a chemotherapy drug, so significant side effects occur. Oral and eye problems are most common; the most serious warning concerns the lungs.

Reported adverse events and approximate frequencies (%) 20 40 60 80 Mouth sores (stomatitis) 60–70 Nausea 55–65 Fatigue 40–55 Dry eyes / eye irritation 40–50 Hair loss 30–40 Vomiting 25–30 Diarrhea 20–30 Conjunctivitis 20–25 Low neutrophil count 15–20 Lung disease (ILD) 3–8 — the most serious warning Rates may vary between study populations and include events of all grades.
The most frequent adverse events affect the mouth and eyes; however, interstitial lung disease is clinically the most critical despite its lower frequency.
Key Safety Issues
  • Interstitial Lung Disease (ILD) / Pneumonitis: This is this ADC class's most serious and potentially fatal adverse effect. It is uncommon, but consequences can be severe. New or worsening cough, shortness of breath or fever must be reported promptly. If moderate or more severe ILD develops, treatment is permanently discontinued.
  • Bone Marrow Suppression: Low neutrophil, platelet and red blood cell counts may occur; regular blood count monitoring is required.
  • Ocular Toxicity: Dry eyes, conjunctivitis and corneal problems. Preservative-free lubricating drops are recommended, and contact lenses should not be worn during treatment .
  • Embryo-Fetal Toxicity: DXd is genotoxic and poses a risk of serious harm during pregnancy. Effective contraception is required during treatment and for a specified period afterward; breastfeeding is not recommended.
  • Infusion Reactions: Uncommon; observation follows administration during the first cycles. Premedication is given to prevent nausea, vomiting and reactions.
Practical Management: Two Simple Measures Make a Major Difference

For Mouth Sores: The drug's most common side effect is oral inflammation, and it is preventable. Starting before treatment, the standard approach is to use an alcohol-free steroid mouthwash and hold ice chips in the mouth during infusion. These simple measures substantially reduce symptom severity.

For the Eyes: Several times a day, preservative-free artificial tears should be used, and contact lenses avoided . Visual changes or persistent redness require ophthalmological assessment.

RESISTANCE MECHANISMS: WHY DOES TREATMENT LOSE EFFECTIVENESS?

Resistance to antibody-drug conjugates can develop when any link in the chain fails:

  • Loss of the Address: On the tumor cell surface, TROP2 levels decrease or disappear — the antibody cannot find its binding target.
  • Failure to Open the Door: Even if binding occurs, impaired internalization through endocytosis prevents delivery.
  • Failure to Release the Payload: If the lysosomal environment is disrupted, the linker is not cleaved and DXd cannot be released; the drug cannot become active.
  • Payload Efflux: Increased cellular toxin-efflux systems may prevent DXd from reaching adequate levels.
  • Resistance at the Target Enzyme: Topoisomerase I alterations or increased DNA repair capacity may compensate for the damage.
  • Tumor Microenvironment: Hypoxic, acidic conditions, dense connective tissue and extracellular matrix may physically impede drug delivery to the tumor.
DRUG INTERACTIONS AND SPECIAL SITUATIONS
Situation Points Requiring Attention
CYP3A4 interactions The cytotoxic payload DXd is metabolized mainly through hepatic CYP3A4. Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, itraconazole) may increase DXd levels and toxicity; strong inducers (e.g., rifampicin, carbamazepine, phenytoin) may reduce effectiveness.
P-glycoprotein DXd is thought possibly to be a substrate of this transporter; caution is required with strong inhibitors.
Other bone marrow-suppressing drugs Datroway is used as monotherapy. If other cytotoxic drugs are used with it, the risk of low blood counts increases.
Hepatic impairment Because DXd is metabolized in the liver, caution is required. Data in moderate and severe impairment are limited; assess individually.
Renal impairment Generally no adjustment in mild–moderate impairment; data in severe impairment are limited, with reports of increased serious adverse events.
Pregnancy and breastfeeding Not used during pregnancy because of embryo-fetal risk; effective contraception and stopping breastfeeding are recommended.
How to Read This Monograph Correctly
  • Datroway is not authorized or available in Turkey. There is no SGK reimbursement or Turkish price. This monograph is informational and does not recommend or direct procurement in Turkey.
  • Evidence Strength Differs by Indication. The first-line TNBC approval rests on survival data; the study supporting the HR-positive/HER2-negative indication did not demonstrate an overall survival benefit.
  • The Lung Cancer Indication Has Accelerated Approval. It is response-rate-based; continued approval depends on the confirmatory study and may be withdrawn.
  • This Is Not a Treatment Recommendation. Dosage information is deliberately omitted. Administration, dose and duration are determined only by current product information and the treating oncologist.
  • Prices Are List Prices and may not reflect actual payments; they vary by country, insurer and agreements.
  • Approval and Authorization Statuses Can Change. This monograph is current as of July 2026; consult your physician about the latest status.
For Patients and Families: A Practical Summary

Datroway Is Not Currently Available in Turkey. This is direct, clear information for patients researching the topic, but not a reason for hopelessness. Another treatment targeting TROP2, sacituzumab govitecan , is authorized in Turkey, and the field of antibody-drug conjugates is expanding rapidly. Other effective, approved and accessible options also exist for hormone receptor-positive metastatic breast cancer .

Approval abroad does not mean a drug is the best treatment for you. Selection depends on cancer subtype (HR/HER2 status, triple negativity), prior treatments, disease extent and overall health. No treatment should be started, stopped or changed on the basis of this monograph.

Questions to Ask Your Oncologist: “Is TROP2-targeted treatment appropriate for my cancer subtype? Which options are accessible in Turkey? What survival evidence supports them? Is there a clinical trial suitable for me?”

DROZDOGAN AKADEMİ COMMENTARY
⚡ Why Does It Matter?

Datroway exemplifies the expanding role of TROP2-targeted ADCs in breast and lung cancer. With first-line TNBC approval in May 2026, this class has begun to replace chemotherapy in patients ineligible for immunotherapy (NCCN Category 1 preferred).

✅ What Do We Know?

Three FDA indications: HR+/HER2− breast cancer (January 17, 2025, after endocrine therapy and chemotherapy), EGFR-mutated NSCLC (June 23, 2025, accelerated), and first-line TNBC (May 22, 2026, ineligible for immunotherapy). TROPION-Breast02 showed PFS of 10.8 vs 5.6 months and OS of 23.7 vs 18.7 months. Approved in Europe since 2025. US list price approximately 4891 dollars per vial.

⚠️ Limitations and Controversies

No Authorization in Turkey — unavailable in the country, with no reimbursement or price. TROPION-Breast01 failed its co-primary OS endpoint (18.6 vs 18.3 months; HR 1.01) but received full approval; literature described this as a controversial regulatory precedent. The lung indication is accelerated. High stomatitis and ocular adverse-event rates; uncommon but serious ILD risk.

🩺 Implications for Practice

No direct effect on current Turkish practice because access is unavailable. However, TROP2-targeted ADC sequencing is changing rapidly, and authorized alternatives such as sacituzumab govitecan exist in Turkey. Where used, proactive oral care (steroid mouthwash and ice during infusion) and eye protection are standard; respiratory symptoms warrant a low threshold for imaging.

❓ Open Questions

When will the Turkish authorization application be resolved? Will longer follow-up change HR-positive survival data? Will the accelerated lung approval be confirmed? How should TROP2 ADCs (Datroway, sacituzumab govitecan, sacituzumab tirumotecan) be selected and sequenced? Can patients respond to another ADC after one ADC?
