ASCO Updates Guidelines for Stage 4 Non-Small Cell Lung Cancer

ASCO Updates Guidelines for Stage 4 Non-Small Cell Lung Cancer
In this article

The American Society of Clinical Oncology (ASCO) has published new guidelines for stage IV non-small cell lung cancer (NSCLC). Under the updated recommendations, anti-PD-(L)1 agents, either alone or with platinum-based chemotherapy, are not recommended for patients with NSCLC whose disease progresses during EGFR tyrosine kinase inhibitor (TKI) treatment. This is a strong recommendation supported by high-quality evidence.

Recommendations for Patients Without Driver Mutations

For patients with stage IV NSCLC without driver mutations, the only updated recommendationis that patients with HER2 overexpression (IHC 3+) and good performance status may receive the following as a second-line or later treatment option: trastuzumab deruxtecan (Enhertu) . However, this recommendation is based on low-quality evidence.

Updated Recommendations for Patients With Driver Mutations

For patients with stage IV NSCLC and driver mutations, the updated guidelines include new recommendations for first- and second-line treatment.

  • Patients With an EGFR Exon 19 Deletion or Exon 21 L858R Mutation:

  • Patients Whose Disease Progresses After Osimertinib or Other EGFR-TKI Treatments:

    • For patients without T790M or other targetable mutations, platinum-based chemotherapy, with or without amivantamab , is recommended. (This is a strong recommendation based on moderate-quality evidence.)

    • If the patient has not received amivantamab and chemotherapy after EGFR-TKI treatment, chemotherapy alone ± anti-VEGF treatment combinations may be considered. This option is recommended particularly for patients with adenocarcinoma histology in whom anti-VEGF treatment is considered safe.

The Role of Immune Checkpoint Inhibitors

According to the ASCO guidelines, anti-PD-(L)1 agents, either alone or with platinum-based chemotherapy, are not recommended for patients whose disease progresses during EGFR-TKI treatment. This is a strong recommendation based on high-quality evidence and indicates that immunotherapies are not effective for this patient population.

Conclusion and Clinical Significance

These updated ASCO guidelines introduce important changes to individualized treatment plans for patients with stage IV NSCLC. The strong warning against immunotherapy use in patients receiving EGFR-TKI treatment is particularly notable. Adding trastuzumab deruxtecan as an option for patients with HER2 overexpression also offers a new treatment alternative for a rare but important patient population.

These updates will help support evidence-based best practices in NSCLC treatment and identify the most appropriate approaches for patients.
