Cisplatin, Gemcitabine and Durvalumab Treatment Protocol for Bladder Cancer

In this article
Summary
Cisplatin + gemcitabine + durvalumab (Imfinzi) is approved for muscle-invasive bladder cancer in perioperative use: neoadjuvant treatment with durvalumab + cisplatin/gemcitabine is followed by radical cystectomy , then adjuvant treatment with single-agent durvalumab continues.
Drugs and Mechanisms
- Gemcitabine: A pyrimidine analog antimetabolite; inhibits cell division by blocking DNA synthesis.
- Cisplatin: Binds covalently to DNA, inhibiting replication/transcription.
- Durvalumab (Imfinzi): Blocks PD-L1 to reactivate T cells and enhance immune-mediated antitumor responses.
The combination aims for rapid chemotherapy-induced shrinkage and lasting immune control through immunotherapy.
Full Indication (FDA)
In adults with muscle-invasive bladder cancer , perioperative treatment: neoadjuvant durvalumab + cisplatin/gemcitabine (before surgery) and adjuvant durvalumab monotherapy after radical cystectomy. FDA approval date: 28 March 2025.
Eligible Patients / Inclusion Criteria
- Disease Status: T2–T4a, N0 or N1, M0 (no distant metastases).
- Surgical Eligibility: Candidates for planned radical cystectomy.
- Prior Systemic Treatment: Treatment-naive before the perioperative plan (for neoadjuvant treatment).
- Platinum Eligibility: Cisplatin tolerance, with adequate kidney function and performance status.
- Performance Status: Usually ECOG 0–1.
- Age: Adults.
Treatment Goals
- Neoadjuvant: Shrink the tumor, improve pathological complete/deep response and target micrometastatic disease.
- Adjuvant: Reduce postoperative recurrence risk and eliminate possible residual micrometastases.
- Goal: Increase cure chances and extend event-free survival (EFS).
Dosing and Schedule (Perioperative Plan)
Neoadjuvant Phase (Chemotherapy + Immunotherapy)
| Drug | Dose | Day | Administration |
|---|---|---|---|
| Durvalumab | 1500 mg IV | Day 1 | ~1-hour infusion |
| Cisplatin | 70 mg/m² IV | Day 1 | 1–2-hour infusion (alternatively, administration on days 1 and 8) |
| Gemcitabine | 1000 mg/m² IV | Days 1 and 8 | ~30-minute infusion |
Cycle: 21 days = 1 cycle | Total Neoadjuvant Treatment: 4 cycles.
Surgery
Radical cystectomy and pelvic lymph node dissection where appropriate.
Adjuvant Phase (Immunotherapy Alone)
- Durvalumab: 1500 mg IV every 4 weeks (1 dose every 28 days) for 8 cycles.
- Total Duration: Depending on response and tolerance, ≤ 12 months.
Estimated Infusion Times
- Neoadjuvant Day 1: Including premedication and hydration, up to ~4 hours.
- Neoadjuvant Day 8: usually ~30 minutes (if gemcitabine alone is planned).
- Adjuvant Day 1: ~1 hour (durvalumab).
Actual times vary by protocol, premedications and tolerance.

SGK / Status in Turkey
- Gemcitabine & Cisplatin: Reimbursed by SGK.
- Durvalumab (Imfinzi): Licensed in Turkey; however, perioperative muscle-invasive bladder cancer is not yet reimbursed by SGK.
- Durvalumab (Imfinzi) Price: 500 mg vial: 74,425.28 TL; 120 mg vial: 17,856.38 TL.
Special Considerations
Hematological Toxicity
- Neutropenia, thrombocytopenia, anemia; complete blood count required before each cycle.
- Delay cycles/modify doses for low counts; G-CSF if needed.
Kidney Function
- Cisplatin is nephrotoxic; use caution/consider alternatives at Ccr <60 mL/min, with intensive hydration.
Liver Function
- Gemcitabine undergoes hepatic metabolism; adjust doses for elevated AST/ALT/bilirubin.
Immune-Mediated Events (Durvalumab)
- Thyroid disorders, hepatitis, pneumonitis, colitis, dermatitis → corticosteroid management according to guidelines.
Nausea/Emetogenicity
- Cisplatin is highly emetogenic: prophylaxis with NK1/5-HT3/dexamethasone ± olanzapine.
Commonly Reported Side Effects
- Nausea, vomiting, loss of appetite, fatigue.
- Hematological: anemia, neutropenia, thrombocytopenia; fever.
- Laboratory: elevated creatinine (cisplatin); liver enzymes.
- Immune-mediated: thyroid dysfunction, rash/itching, pneumonitis (rare).
- Urinary and gastrointestinal symptoms, such as urinary tract infection, diarrhea or constipation.
Clinical Efficacy (NIAGARA Context)
Adding durvalumab to neoadjuvant chemotherapy and continuing it after surgery significantly improved pathological response measures and event-free survival. Appropriate patient selection aims to increase cure chances.
