Approved Indications
Nerlynx is approved for two main breast cancer indications, as monotherapy or combination treatment:
- Extended Adjuvant Treatment: In adults with early hormone receptor-positive breast cancer and HER2 overexpression/amplification who completed trastuzumab-based adjuvant treatment less than 1 year earlier and are N2-positive (4 or more positive axillary lymph nodes), monotherapy is used for a maximum of 1 year to reduce the risk of recurrence.
- Advanced or Metastatic Disease: Used in advanced or metastatic HER2-positive breast cancer after at least two prior anti-HER2 regimens, combined with capecitabine (chemotherapy) as a treatment regimen.
FDA Approval History
Neratinib completed FDA approval processes based on survival results from international clinical trials:
- July 17, 2017: The FDA approved Nerlynx based on the Phase 3 ExteNET trial for extended adjuvant treatment of early HER2-positive breast cancer.
- February 26, 2020: Additional approval for metastatic HER2-positive disease with capecitabine followed the Phase 3 NALA trial.
- Clinical Study (ExteNET): In this double-blind trial of 2,840 women, Nerlynx reduced invasive disease recurrence or death in the intention-to-treat (ITT) population at the primary 2-year analysis by 33% (HR=0.67, p=0.011). The greatest benefit occurred in the hormone receptor-positive ER/PgR+ subgroup (HR=0.50).
What Is Nerlynx and How Does It Work?
Nerlynx is a small targeted molecule acting on the intracellular portions of cell-surface receptors. Unlike antibody treatments such as trastuzumab, it enters cells and inhibits key enzymes.
In HER2-positive breast cancer, excessive signaling from surface growth-factor receptors drives uncontrolled division. Neratinib interrupts this process as follows:
- Irreversible Blockade: It binds to ATP-binding sites of 3 ErbB family members (EGFR/ErbB1, HER2/ErbB2, HER4/ErbB4) and their heterodimers with HER3 (ErbB3), through covalent, irreversible binding.
- Interrupting Signaling: This covalent binding strongly suppresses receptor autophosphorylation and the downstream MAPK and AKT pathways driving cell growth.
- Cell Death (Apoptosis): Tumor cells stop proliferating without division signals, and programmed cell death is triggered.
Administration and Dosage
The schedule and food requirements are crucial for effectiveness and side-effect management:
- Standard Dose: The recommended daily dose is 240 mg (6 film-coated 40 mg tablets), taken once daily at the same time.
- Metastatic Use: With capecitabine, take 240 mg PO continuously on days 1-21 of each 21-day cycle; capecitabine is given twice daily on days 1-14.
- Food Interaction (Critical): Tablets must be taken with food, not on an empty stomach . A high-fat meal increases systemic exposure (Cmax and AUC) by approximately 100% (2-fold).
- Administration Rules: Swallow tablets whole; do not chew, crush, or dissolve them. Grapefruit, pomegranate, and their juices may increase plasma concentrations and during treatment must not be consumed.
- Missed Doses: Do not double doses; resume the normal schedule the next day. In severe hepatic impairment (Child-Pugh C), the maintenance dose is reduced to 80 mg.
Pharmacokinetic Properties
Absorption, distribution, and elimination are as follows:
- Absorption and Distribution: Oral absorption is slow, reaching peak plasma concentration (Tmax) in about 7 hours. Binding to plasma proteins, mainly albumin and alpha-1 acid glycoprotein, exceeds 98%and is extensive.
- Metabolism (Biotransformation): Metabolism occurs mainly in the liver through CYP3A4 microsomal enzymes and, to a lesser extent, flavin-containing monooxygenase (FMO). There are 4 major circulating metabolites with similar potency: M3, M6, M7, and M11.
- Excretion: The main elimination route is fecal. Of a radiolabeled dose, 95.5% is excreted in feces, and only 0.96% through urine. No adjustment is therefore needed for mild or moderate renal impairment. Mean apparent plasma half-life is 17 hours.
Resistance Mechanisms
With prolonged treatment, tumor cells may develop defenses to evade the drug:
- Target Receptor Mutations: Specific HER2/neu ATP-binding pocket mutations, such as the T798I gatekeeper mutation, may reduce covalent binding affinity and cause resistance.
- Enzymatic Upregulation: Excess CYP3A4 activity in the liver or tumor microenvironment may accelerate metabolism and reduce therapeutic effectiveness.
Drug Interactions and Management
Caution: Neratinib is a strong CYP3A4 metabolism substrate and P-glycoprotein (P-gp) inhibitor. Concurrent medications may cause serious toxicity or treatment failure:
CYP3A4 inducers reduce exposure by up to 87%. Their use is contraindicated:
- Strong: Rifampicin, Phenytoin, Carbamazepine, St. John's Wort (Hypericum perforatum)
- Moderate (Not Recommended): Dexamethasone, Phenobarbital, Bosentan, Efavirenz
CYP3A4 inhibitors increase drug levels up to 4.8-fold. If unavoidable, reduce Nerlynx dosage:
- Strong: Ketoconazole, Itraconazole, Clarithromycin, Voriconazole, Ritonavir, Cobicistat
- Moderate: Erythromycin, Fluconazole, Diltiazem, Verapamil, Ciprofloxacin
Solubility is pH-dependent; higher gastric pH reduces absorption:
- Proton Pump Inhibitors (PPIs): Concurrent use with omeprazole, lansoprazole, and similar agents is not recommended.
- H2 Receptor Antagonists: Nerlynx should follow an H2 blocker by at least 10 hours or precede the next H2 blocker dose by at least 2 hours when scheduling doses.
- Antacids: Separate antacids and Nerlynx by at least 3 hours between doses.
Inhibiting P-gp and BCRP transporters may increase toxicity of drugs transported through these pathways. Patients taking digoxin, colchicine, dabigatran, cyclosporine, everolimus, rosuvastatin, or irinotecan require close monitoring. Increase PT/INR monitoring in warfarin users.
Side Effects and Toxicity Management
Gastrointestinal and dermatological reactions dominate the adverse-effect profile. Prophylaxis is crucial.
Diarrhea occurs in 93.6%, with 36.9% of cases Grade 3. The median time to the first episode is treatment day 2, and episodes concentrate in the first month. Diarrhea leads 14.4% to discontinue permanently and 24.7% to reduce dosage.
Management: Proactively begin loperamide prophylaxis on the first treatment day and ensure ample fluid intake against dehydration.
Nausea (42.5%), vomiting (26.8%), abdominal pain (22.7%), reduced appetite (13.7%), and stomatitis or aphthous ulcers (11.2%) are very common. Symptomatic antiemetics may be used.
Clinical studies found significantly more ALT (8.5%) and AST (7.4%) elevations in the Nerlynx arm than placebo. Grade 3-4 elevations may occur. During the first 3 months, and then at routine intervals, liver function tests (LFTs) should be checked, twice monthly during the initial period.
Rash occurs in 15.4%, including erythematous, pustular, or acneiform reactions; 7.8% have nail abnormalities such as paronychia, discoloration, or onychoclasis. Pooled data show skin and nail toxicity in Asian patients (57.1%)significantly more often than in Caucasian patients (34.6%).
Special Populations and Precautions
- Older Patients (65 and Above): Discontinuation for adverse reactions is substantially higher than in younger patients (44.8% versus 25.2%). Monitor vomiting, dehydration, and acute kidney failure closely.
- Pregnancy and Contraception (Category D): Animal studies found embryo-fetal death and abnormalities. Women of childbearing potential must use highly effective contraception during treatment and for 1 month afterward . Because Nerlynx may reduce hormonal contraceptive effectiveness, add a barrier method (condom) . Men must use barrier contraception during treatment and for 3 months after the last dose.
- Lactation (Breastfeeding): Passage into human milk is unknown. Because risk to the infant cannot be excluded, breastfeeding during treatment is not recommended.
- Driving and Operating Machinery: Weakness, fatigue, dizziness, and dehydration-related syncope may mildly or moderately impair driving or machinery use.
