In this article
Sutent (Sunitinib) Profile
- FDA Approval: Yes (First approval: January 26, 2006)
- Brand Name: Sutent
- Active Ingredient: Sunitinib malate
- Administration: Oral (capsule taken by mouth)
- Class: Tyrosine kinase inhibitor (TKI)
- Category: Antineoplastic / Targeted therapy
- Manufacturer: Pfizer Inc.
- Availability in Türkiye: Licensed in Türkiye and covered by SGK.
- Approved Cancers:
- Metastatic renal cell carcinoma (RCC)
- Gastrointestinal stromal tumor (GIST), resistant or intolerant to imatinib
- Pancreatic neuroendocrine tumors (pNET)
What Is Sunitinib? How Does It Work?
Sutentis an oral targeted cancer medicine containing sunitinib malate. It is a tyrosine kinase inhibitor (TKI) used particularly for kidney cancer, GIST, and certain neuroendocrine tumors.
Mechanism of Action: Sutent blocks several tyrosine kinase enzymes required for cancer-cell growth and blood vessel formation around tumors (angiogenesis).
Main receptors targeted:
- VEGFR: Stops new blood vessel formation (angiogenesis).
- PDGFR: Prevents tumor spread into supporting tissues.
- c-KIT: Blocks the receptor driving GIST cell growth.
- FLT3, RET, and CSF-1R: Targets other critical kinases, making tumor survival harder.
FDA (US Food and Drug Administration) Approval History
- November 16, 2017: Approved in high-risk adult renal cell carcinoma patients for adjuvant treatment.
- May 20, 2011: Approved for a rare pancreatic cancer type (pNET).
- January 26, 2006: Received initial approvals for both GIST and advanced renal cell carcinoma (RCC).
- August 10, 2005: Pfizer submitted a new drug application to the FDA.
Licensing in Türkiye and Therapeutic Indications
Licensed Uses:
- For metastatic or locally advanced, inoperable disease with a Ki-67 index of 5 or below, showing progression by RECIST after somatostatin and one line of chemotherapy, in pancreatic neuroendocrine tumors .
- Treatment of imatinib-resistant metastatic/unresectable GIST or metastatic renal cell carcinoma.
Indications Usable Without Additional TİTCK Approval:
- Metastatic adenoid cystic carcinoma
- Advanced chondrosarcoma (after 1st-line treatment)
- Solitary fibrous tumor (2nd line and later)
- Alveolar soft part sarcoma
- Hemangiopericytoma (after 1st-line treatment)
- Advanced chordoma (after imatinib)
- Soft tissue sarcoma (after progression)
SGK Reimbursement and Pricing
Sunitinib is covered by SGK reimbursement in Türkiye.
December 2025 Pharmacy Retail Prices:
- Sutent 12.5 mg capsule: 13,657.15 TL
- Sutent 25 mg capsule: 27,185.39 TL
- Sutent 37.5 mg capsule: 40,713.66 TL
- Sutent 50 mg capsule: 27,185.08 TL
- Average cycle cost: 27,185.08- 40,713.66 TL.
Resistance Mechanisms
- Alternative Angiogenesis Pathways: Tumor cells activate bypass pathways such as FGF (fibroblast growth factor) .
- Target Kinase Mutations: Structural changes in VEGFR, c-KIT, or PDGFR receptors.
- Efflux Pumps: Increased P-glycoprotein (P-gp) pumps the drug out of cells.
- EMT and Stem Cells: Cells adopt a more aggressive phenotype, and resistant stem cells cause recurrence.
Pharmacokinetic Information
| Absorption | Bioavailability >50%. Time to Cmax: 6-12 hours. No food interaction. |
| Metabolism | Metabolized in the liver by CYP3A4 . Its main active metabolite, SU12662 , is at least as effective as the drug itself. |
| Half-Life | Sunitinib: 40-60 hours / SU12662: 80-110 hours. |
| Excretion | 60% is eliminated in feces and 15-20% in urine. |
Dosage and Treatment Regimens
- mRCC and GIST: 50 mg daily. Regimen: 4 weeks on treatment + 2 weeks off (4/2 cycle).
- pNET: 37.5 mg daily. Continuous (uninterrupted) daily use.
- Adjuvant RCC: 50 mg daily (4/2 regimen). Total 9 cycles (approximately 1 year).
Drug Interactions and Warnings
- CYP3A4 Inhibitors: Ketoconazole, erythromycin, and grapefruit juice increase blood drug levels and toxicity.
- CYP3A4 Inducers: Rifampicin, phenytoin, and dexamethasone may reduce efficacy.
- Cardiac: Arrhythmia risk increases with QT-prolonging drugs such as amiodarone.
- Other: Use caution with warfarin (bleeding risk) and levothyroxine (development of hypothyroidism).
Situations Requiring Attention
- Hypertension and the Heart: Blood pressure monitoring is essential; left ventricular function should be monitored.
- Hepatotoxicity: Check AST, ALT, and bilirubin regularly.
- Osteonecrosis of the Jaw: A dental examination is required before dental surgery.
- Wound Healing: Stop the drug at least 2 weeks before surgery.
- Thyroid: Monitor TSH for development of hypothyroidism.
