Major Advance in Cancer: Living Drug CAR T-Cell Therapy Receives FDA Approval!
In this article

Cancer is caused by gene damage. Advances in genetics have enabled treatments based on genetic engineering. The most advanced genetic treatment is CAR T-cell therapy. Yesterday brought the field's most exciting development. The United States Food and Drug Administration, FDA, tisagenlecleucel (marketed as Kmyriah) was approved as a CAR-T therapy for children and young adults with B-cell acute lymphoblastic leukemia (ALL).

The 10 Hallmarks of Cancerinclude learning to become invisible to immunity. Cancer cells display proteins making them appear friendly to T cells, escaping attack. CAR T-cell therapy is a form of cancer immunotherapy that changes T-cell receptors to attack cancer more effectively. In other words, genetic techniques can produce billions of T cells capable of recognizing cancer cells.

For Which Cancer Will Kmyriah Be Used?

It is approved for pediatric and young adult ALL patients. Those no older than 25 with refractory or relapsed B-cell ALL are eligible.

What Is B-Cell Acute Lymphoblastic Leukemia?

Leukemias are hematological cancers originating in blood and bone marrow, popularly called blood cancer . Acute leukemias fall into AML and ALL as 2 main groups, each with nearly 10 subtypes. All are rapidly progressing cancers. AML is commoner in adults, whereas ALL is commoner in children and young adults.

In lymphoblastic or acute lymphoid leukemia (ALL), white blood cells begin rapid, uncontrolled bone marrow proliferation while still at the preceding developmental stage, lymphoblast . Cancerous lymphoblasts invade marrow, reducing white cells, red cells and platelets and causing infection, bleeding and fatigue. Leukemia cells accumulate in lymph nodes, liver and spleen, enlarging these organs.

What Are the Types of ALL?

  • B-cell ALL: %75 of patients
  • T-cell ALL: %20 of patients
  • Burkitt ALL: %5 of patients

What Is CAR T-Cell Therapy and How Is It Performed?

what is CAR T how does this cancer immunotherapy work

CAR T-cell therapy entirely personalized is the nature of this treatment. The patient's T cells are genetically trained to attack cancer. First, T cells are isolated from a blood sample. Laboratory gene transfer enables them to recognize cancer, and modified cells are returned to the patient. Producing this individualized drug takes approximately 22 days.

Because living cells are infused into the body, it is also called a living drug in this context.

Which Clinical Trials Supported CAR T Approval?

Safety and efficacy were examined in multicenter clinical trials. Approximately 150 patients participated in 3 trials, with complete disappearance of disease signs measured as remission rates of %70 and %95. Deaths occurred in 17 patients, 14 from the disease itself and 3 from infection.

FDA approval was based on phase-2 ELIANA trial findings. Among 63 pediatric and young adult refractory or relapsed ALL patients, %83 achieved complete response within 3 months of CAR T treatment.

What Are CAR T-Cell Therapy's Side Effects?

Side effects can be dangerous. Cytokine release syndrome occurred in 60 percent of trial patients: CAR T cells release excessive cytokines, severely affecting blood vessels and causing high fever and sudden hypotension. No deaths from this effect occurred in the trials. The FDA also approved a medicine treating this CAR T-related effect, tocilizumab (Actemra), for patients aged 2 and above. In trials, cytokine release syndrome resolved in 69 percent of CAR T recipients after one or 2 Actemra doses within 2 weeks.

Other effects include hypotension, acute kidney failure, high fever and hypoxia. Most occur between 1 and 22 days after Kymriah infusion. CAR T therapy also destroys antibody-producing B cells, creating long-term infection risk.

What Does This Development Mean for Cancer?

  • Cancer treatment has taken a historic step . For the first time, gene therapy will be used in B-cell acute lymphoblastic leukemia.
  • Although currently used for one disease, CAR T promises benefits in other cancers. The FDA reports 76 CAR T drug applications. More than 500 gene therapy products are also being tested in cancer, autoimmune diseases and diabetes.
  • Many scientific authorities consider this one of the century's most important medical advances.
  • Concerns remain. Cancer cells can rapidly undergo mutations , so resistance may develop in some patients. Patients may deteriorate while new immune cells are prepared.
  • Cost is another issue. Highly precise production using advanced genetic engineering makes treatment expensive: 475.000 dollars per patient. Licensed producer Novartis says patients not responding after the first month will not be charged.
  • Novartis plans to offer treatment at 32 centers worldwide. For safety, only specially trained and certified hospitals can deliver this complex therapy.
  • Many cancer research centers worldwide are developing CAR T for solid tumors such as brain, breast and pancreatic cancer.

Modern cancer therapies are developed through multidisciplinary work and highly rigorous safety and efficacy testing. Cancer can be treated only through modern medicine's opportunities.

What Is Cancer Immunotherapy? Immunotherapy Types — Further Reading

Immunotherapyis a relatively new medical field effective in many cancers. Its principle is using the patient's own immune cells against cancer.

As the figure shows, there is not just one type. Particularly since 2010, immune checkpoint inhibitors and cellular therapies have become prominent cancer immunotherapies. CAR T belongs to the cellular therapy group.
