What Are the Symptoms of Stage 4 Bladder Cancer?

What Are the Symptoms of Stage 4 Bladder Cancer?
In this article

Stage 4. cancer is difficult to eliminate completely. The main treatment goal is therefore to provide the best possible quality of life and treatments that prolong survival.

Diagnosing cancer, planning treatment, and explaining its status clearly involve staging. Most solid cancers have four stages expressed in Roman numerals, so stage 4. may be written in medical literature as stage IV .

Knowing what to expect from stage 4 bladder cancer can help people make informed treatment decisions.

What Does Stage 4. Bladder Cancer Mean?

Stage indicates where cancer cells are and how far they spread. Some cancers have stage 0, but most use 1 through 4. Stage 4 is advanced; cells generally leave the original tissue and spread to other organs. Stage is usually determined at diagnosis; for example, stage 4 at diagnosis does not later change with treatment.

Three key factors are considered:

  • Primary tumor size and location
  • Whether cancer has spread to lymph nodes near the bladder through metastasis or not
  • Whether it has spread to other organs or distant lymph nodes

Bladder cancers usually start in the innermost layer. By stage 4., the primary tumor has often grown beyond it. Common metastatic sites are the pelvic bone, other bones, lungs, and liver.

what is stage 4 metastatic bladder cancer

Stage 4. bladder cancer is divided into IVA and IVB. In stage IVA, cancer has spread from the bladder to (a) the abdominal or pelvic wall or (b) lymph nodes above the main iliac arteries. Stage IVB’ involves spread to (c) other body parts such as lungs, liver, or bones.

Signs and symptoms include:

  • fatigue or weakness,
  • pain when urinating,
  • difficulty or inability to urinate,
  • lower back pain on one side,
  • weight loss,
  • swollen feet,
  • bone pain,
  • blood in urine (also an early bladder cancer symptom).

How Is Stage 4. Bladder Cancer Treated?

In recent years, immunotherapies entering oncology have sometimes produced exceptionally good results, but stage 4. cancers usually cannot be completely cured. Treatment aims to improve how people feel and give them more time. It may reduce and sometimes eliminate cancer-related symptoms.

Chemotherapy

For stage 4 bladder cancer, the most common treatment, which slows cancer-cell growth or shrinks a tumor, is chemotherapy. Neoadjuvant chemotherapy sometimes enables surgeons to remove part or all of the bladder. Preferred regimens are cisplatin -based.

Immunotherapy

Immune checkpoint inhibitor immunotherapies are particularly effective in bladder cancer. They are considered when advanced patients are unfit for cisplatin-based chemotherapy or progress afterward. FDA-approved advanced bladder cancer immunotherapies, in approval order, are:

  • Atezolizumab (Tecentriq): first approved bladder cancer immunotherapy (2016), as second-line treatment.
  • Nivolumab (Opdivo): 2. line treatment
  • Pembrolizumab (Keytruda): 2. line treatment
  • Avelumab (Bavencio): 2. line treatment
  • Durvalumab (Imfinzi): 2. line treatment

Palliative Care

Palliative care is usually an important part of stage 4 treatment, offering pain relief, emotional support, and advice. It helps patients decide how to manage symptoms and remain as comfortable and pain-free as possible.

Survival Rates and Life Expectancy

Cancer survival rates are estimates only; many individual factors must be considered.

Relative survival compares bladder cancer patients with people of the same age and sex in the general population, potentially giving a more accurate estimate.

For stage 4 bladder cancer, the 5-year relative survival rate is approximately 5 percent. This means a patient’s likelihood of living at least 5 years after diagnosis is 5 percent (these figures are estimates from United States statistics). They use data from 2008–2014. Scientists constantly improve treatment, often improving survival. Immunotherapy had not yet entered bladder cancer treatment during those data years.

Remember that survival rates are estimates and everyone differs. Age and general health, among other factors, can importantly determine how long someone lives with the disease.

Nutrition in Bladder Cancer

Cancer often causes fatigue, and both cancer and treatment can cause weight loss. Small, frequent meals may maintain energy. A healthy diet includes whole grains, fruit, vegetables, some protein, and little or no sugar and saturated fat.

Light Exercise

Light/moderate exercise is safe for most people with cancer, but consulting a doctor first is best. Walking and swimming place little strain on the body and may improve mood and reduce fatigue.

Pain Relief

Pain relief is an important part of treatment for many. Pain differs between people, so describe it to the doctor as clearly as possible. This enables appropriate, fast, effective pain medication.

Conclusion

Living with stage 4 bladder cancer and deciding about treatment can be difficult. Learning as much as possible is generally helpful.

Reliable websites, besides healthcare professionals, can be excellent information sources. Friends, family, or others with stage 4 bladder cancer may provide emotional support.

Stage 4 bladder cancer differs for everyone, and survival rates are estimates. Treatment is often very effective in easing symptoms and providing good quality of life.
